The Comparision Of Curative Effect Between Intraspinal Tumors And Tuberculosis In Lower Thoracic And Lumbar By Different Internal Fixations

Objective:By comparing the Curative Effect between Intraspinal Tumors and Tuberculosis in Lower Thoracic and Lumbar by Different Internal Fixations, to find the best diagnosis and Differential Diagnosis way in various kinds lowback illnesses .Method:By Retrospective study 17 patients from march 2005, to december 2009 in our hospital whose diagnose are intraspinal tumors in lower thoracic and lumbar spine. Among these 10 are male,7 are female.Ages from 13y to 72y, Average age is 26y.,In this group ,9 cases tumors are in the back side, by Posterior excision +RF internal fixation, 9 cases tumors are in the veutro side ; by anterior excision+STS internal fixation. After the operation the patients bed rest.2 weeks later rise and movable by the protect of brace.Another group, 14 cases are tuberculosis in lower thoracic and lumbar spine. Among these 8 are male,6 are female; Ages from 15y to 69y, age between 15 and 69, the average age is 43y.This group all patients give regular anti-TB remedy for 2~4weeks.Among these 7 cases give Anterior debridement +STS internal fixation , 7 cases give Anterior debridement +RF internal fixation, then keep on regular anti-TB remedy for 12 ~ 18 months. After the operation lying bed for 2weeks,then rise and movable by the protect of brace.According to the the clinical of Intraspinal tumor and tuberculosis, refer to the lumbocrural pain curative effect standard formulate by Japanese orthopedics institute in 1984, give the patients recent (2weeks) Follow-up and scoring. Comparing JOA scores before and after the operation, to comparing curative effect between different operative method. Outcome:Two groups of patients symptoms remit a lot after the operation. All patients take out stitches in 2 weeks after surgery, take X-ray examine after a week, screw-rod system in good location. No patients die or infection. In tumors group , posterior approach patients got 23.4 points before and 10.2points after the operation, anterior approach patients got 22.4 points before and 11.3 points after the operation. In TB group , posterior approach patients got 18.6 points before and 2.3points after the operation, anterior approach patients got 19 points before and 1.7 points after the operation. ConclusionIn tumors group, the scores difference between two Operative method have not statistically significant ,It means that Operative method can't effect the Prognosis; In TB group, the scores difference between two Operative method have not statistically significant ,It means that Operative method can't effect the Prognosis.All cases the scores difference between two Operative method have not statistically significant.
