Purpose:To study the effect of different path of administration to use nicorandil on patients with unstable angina and evaluate the efficacy of nicorandil and nitroglycerin.Methods:In this article, we retrospected and followed up 116 cases of unstable angina from January in 2008 to November in Chinese PLA 208 Hospital,The patients with unstable angina were divided into three groups:per os nicorandil group (n=51), intravenous nicorandil group (n=34) and control group (n=31).We compared the following factors among these three groups:age, Braunwald' scale, surveying angina, surveying the resting electrocardiogram after on the lth day,7th day and the 14th day after using medication, following up to survey Cardiovascular events during six months,the adverse reaction at any time.Results:There isn't statistically significant difference in age, Braunwald' scale among the three groups.The data of the three groups are comparable.In our study,The efficiency on control of angina in per os nicorandil group,intravenous nicorandil group and control group) are 58.8%(30/51)%,61.7%(21/34) and 35.4%(11/31) respectively. there is significant difference between per os nicorandil group and control group,there is significant difference between intravenous nicorandil group and control group,But there is no significant difference between per os nicorandil group and intravenous nicorandil group.the efficiency on the improvement of the resting electrocardiogram after on the lth day in per os nicorandil group,intravenous nicorandil group and control group) are 11.7%(6/51),14.7%(5/34) and 9.6%(3/31) respectively. There isn't statistically significant difference among the three groups.the efficiency on the improvement of the resting electrocardiogram after on the 7th day in per os nicorandil group,intravenous nicorandil group and control group) are 27.4% (14/51),29.4%(10/34) and 6.4%(2/31) respectively. there is significant difference between per os nicorandil group and control group,there is significant difference between intravenous nicorandil group and control group,But there is no significant difference between per os nicorandil group and intravenous nicorandil group. the efficiency on the improvement of the resting electrocardiogram after on the 14th day in per os nicorandil group,intravenous nicorandil group and control group) are31.3% (16/51),35.2%(12/34) and 9.6%(3/31) respectively. there is significant difference between per os nicorandil group and control group,there is significant difference between intravenous nicorandil group and control group,But there is no significant difference between per os nicorandil group and intravenous nicorandil group.The cardiovascular events in per os nicorandil group,intravenous nicorandil group and control group) are 3.9%(2/51),2.97%(1/34) and 3.2%(1/31) respectively during six months. There isn't statistically significant difference among the three groups.The adverse reaction in per os nicorandil group,intravenous nicorandil group and control group) are 13.7%(7/51),14.7%(5/34) and 16.1%(5/31) respectively at any time. There isn't statistically significant difference among the three groups.Conclusion:The continuous intravenous use of nicorandil on the unstable angina shows more efficient on resting electrocardiogram and better control of clinical symptoms, and a lower trend in the incidence of low blood pressure. |
