Objective: 1. Use Rradiofrequency ablation (RFA) in the thyroid gland of pig in order to find out the relations between the time, power and area of radiofrequency ablation.2. Use RFA to treat patients who with substantial disease of thyroid gland and need surgery resect subtotal (or total) thyroid with in the intraoperative, To observe the biological effects of Radiofrequency Ablation be used in substantial disease of thyroid gland.Methods: 1.Animal experiment: there are 30 fresh pig`s thyroid tissue were randomly divided into 6 groups, group name are T1,T2,T3,T4,T5,T6,RF electrode was inserted into the each pig thyroid tissue orthoptic and treated for the special time and the special power. To incision each pig thyroid tissue along the direction with the monopolar RF electrode after RFA.RFA lesion areas was measured with its longitudinal length R1 and transverse length R2 , and cross-section area to measure the scope of radiofrequency ablation lesion, record the data in detail. Used SPSS13.0 for all statistical analysis of the data.2.Clinical applied research:Choosed 12 patients biopsy-proven substantial disease of thyroid in our hospital from May to December in 2008.The minmal diameter of substantial disease of thyroid gland must big than 3cm and the site of the disease must in front of the thyroid gland.RF electrode was inserted into the site of disease orthoptic and treated for 10 minutes. The thyroid gland ablation lesions were made by means of radiofrequency and the monopolar electrode with the exposed tip diameter of 0.1cm and tip length of 1cm.The size of the lesion areas was measured and its pathology was studied. All the patients were observed if they have symptoms such as hoarseness,choke,dyspnea,skin burn,infection and so on.Results: 1.Results of animal experiment: Used 5W power to RFA the pig`s thyroid tissue with 1min, 5min and 10min, it can made the average size of the largest cross-section of the lesion areas was 0.154±0.045cm2,0.388±0.087cm2 and 0.589±0.129cm2。Used 10W power to RFA the pig`s thyroid tissue with 1min, 5min and 10min, it can made the average size of the largest cross-section of the lesion areas was 0.598±0.077cm2,1.32±0.113cm2 and 1.77±0.117cm2。2.Results of clinical applied research: The lesion area in the thyroid gland ranged from 0.94 cm2 to 1.53cm2 and the average is 1.18±0.19cm2. The thyroid gland became harder and of white in color after ablation.The coagulation necrosis in the lesions was showed with histological methods under light microscopy.All the patients have no syndromes.Conclusion: 1.To some extent,the size of the lesion areas is determined by the exposed tip`s length of the monopolar electrode. 2.Using the monopolar electrode with the exposed tip diameter of 0.1cm and tip length of 1cm RFA is safety,if the substantial disease of human thyroid gland is big enough.
