T.o.p. Cementless Acetabular For Acetabular Reconstruction

Objective:To evaluate the clinical result of Trabeculae Oriented Pattern(T.O.P.) cementless acetabular prosthesis in total hip arthroplasty(THA) by clinical and radiographic outcomes. Methods:THA with T.O.P cementless acetabular cup was used to treat 75 cases(81 hips) from December 2002 to December 2008. A retrospective study was done to analyze clinical efect and X-ray manifestation of 70 cases(76 hips) that were followed up. Clinical evaluation was based on Harris hip score. The radiographical evaluation was performed according to orthotropia images of hip joint to observe acetabular migration and bone changes around the hip. Results:The mean Harris score before operation was 32.5 points(10～56 points) but that after operation increased to 91.5 points(86～100 points) at latest follow up. Radiographical evaluation didn't show osteolysis and the abduction angel of the acetabula was mean 48.1°. There was no revision of a fixed acetabulum component because of pelvic osteolysis secondary to polyethylene wear. No acetabular component was revised because of aseptic loosening or acetabular component migration at the latest follow up. Conclusion:The application effect of T.O.P cementless acetabular cup in THA received good effects, the clinical effects are satisfactory. The adequate preoperotive preparation,well-trained surgical operation technique and the complete rehabilitation training after the operation are the key to the successful. A long term follow up for cementless acetabular prosthesis is necessary because of potential factors like polyethylene wear and osteolysis.
