Efficacy Of Comprehensive Intervention Of Nahc Program Combined With Copc Model In The Treatment Of Hypertension Patients With Different Ages

Objective To evaluate the efficacy of comprehensive intervention of NAHC program combined with community-oriented primary care (COPC) model in the hypertension patients with different ages and provide a reference for the comprehensive community prevention and treatment of future hypertention and other chronic diseases in China.Method Lijun Community in Xiufeng District of Guilin was selected as the study site. 8 neighborhood committees were randomly selected to carry out comparative study of comprehensive intervention of hypertention. The intervention period was 1 year. NAHC program combined with COPC model was applied to selected 631 hypertension patients in the area of 4 neighborhood committees (NAHC+COPC group). The patients of age≥50 were 323, and the patients of age<50 were 308. NAHC program was alone applied to selected 677 hypertension patients in the area of other 4 neighborhood committees(control group). The patients of age≥50 were 346, and the patients of age<50 were 331.Selection criteria was 1 and 2 levels blood pressure of essential hypertention patients who didn't include sick sinus syndrome,second degree or higher atrioventricular block, drug allergy, bronchial asthma, chronic obstructive pulmonary disease, midrange and severe renal insufficiency, gout patients and pregnant and lactating women.Result Compared with the control group, patients in the NAHC+COPC group had higher rates of awareness on 6 hypertension-related issues, higher rates of total efficacy and apparent efficacy in the treatment of hypertension, lower percentage of very high risk patients, and higher percentage of low risk patients. Among the patients of age≥50, NAHC+COPC group had notably higher rates of total efficacy and apparent efficacy, lower percentage of very high and high risk patients, and higher percentage of low risk patients than the control group. But, among the patients of age<50, NAHC+COPC group only showed lower percentage of very high risk patients and higher percentage of low risk patients. In the control group, patients of age<50 had better rates of awareness on 4 hypertension-related issues than those of age≥50, while there was no significant difference in the rates of awareness on all 8 hypertension-related issues between patients of age<50 and patients of age≥50 in the NAHC+COPC group.Conclusion COPC model can notably enhance the rate of awareness on hypertension-related knowledge for hypertension patients. NAHC program combined with COPC model has better clinical efficacy than NAHC program alone in the treatment of hypertension patients in the community, especially for those of age≥50, which have a certain reference value for the comprehensive community prevention and treatment of future hypertention and other chronic diseases in China.
