Objective:To evaluate the clinical efficacy on the sequel of pelvic inflammatory diseases(PID) with traditional Chinese medicine(TCM) more comprehensively and objectively, we built the scales for overall evaluation the TCM clinical effectiveness on sequel of PID in the perspective of TCM syndromes, signs and quality of life(QOL), and to guide the TCM clinical research of PID sequela.Methods:Through literature survey and expert consulting, set up the evaluation scale system including TCM syndromes, signs and QOL of PID. By Cross-section survey, patients of PID sequela from both out-patient and in-patient of gynecology department in Chengdu University of TCM have been surveyed with questionnaires. This study is based on the project of the Eleventh Five-Year Plan Key Medical Units by State Chinese Medicine Administration Bureau, that's the clinical validation of therapeutic regimens in the treatment of PID. All cases were evaluated by the two scales of the project and our study during three timing-piont:before treatment,1 month follow-up,2 months follow-up, and evaluated their QOL by interview. Input data double-checked by Epidata, statisticsed by SPSS16.0, analysied by paired t-test and rank test for final results.Results:This study involved 131 cases,11 of them were excluded and 120 cases were evaluated. By rank test, the evaluation results about percentage of marked effect of the two scales were same(P>0.05). The difference had statistical significance among our overall scales for three treatments on its primary symptoms, minor symptoms and physical sign, but there was no significant difference of the project's scale.There was statistical significance between self-evaluation of quality life by patients and objective evaluation by doctors(P<0.01). With the weight ratio analysis the most influential factor for quality life was repeating lower abdominal and sacroiliac pain, the scores was only 2.61,which was between frequent(1.5625) and sometimes(3.125).The average scores of social relationship was highest and of psychological realm was lowest. The slope of disease remission for 37 cases between groups and Before-after study was much more than the change of scores for quality life.Conclusion:The overall evaluation methods of TCM Clinical Efficacy on PID sequela could be better at reflecting the conditions and changes of patients because it's more objective, comprehensive and sensitive. Surveys shows that repeating lower abdominal and sacroiliac pain bothers patients most,.self-evaluation of quality life by patients was lower than objective evaluation by doctors. QOL instruments are a necessary complement for clinical efficacy evaluation on the relationship among mentality, physiology and social relationships besides diagnosis of traditional Chinese syndrome, physical sign. |
