Clinical Observation On The Treatment Of Small Follicle Ovulation With Kidney - Spleen And Spleen Deficiency By Staging

[Background]Small follicle ovulation, also known as follicular dysplasia, refers to the dominant follicle is not yet mature, ovulation occurs a ovulation obstacle, and follicle quality is not high, the cycle clinical pregnancy rate is low, lead to irregular menstruation, infertility, spontaneous abortion, recurrent spontaneous abortion. The main use of Western medicine treatment is ovulation induction drugs which have many side effects. Chinese medicine treatment has certain advantages, without the side effects of Western medicine. The main pathogenesis is kidney deficiency, spleen deficiency, liver depression. Clinical observation found that hypothyroidism and subclinical thyroid function loss can also cause spontaneous abortion, recurrent spontaneous abortion, is closely related with the female reproductive. After making a literature review, we found that the ovary is also the target organ of thyroid hormone, and thyroid stimulating hormone can affect ovarian function by competing with gonadotropin.[Objective]This study intends to explore the therapeutic effect of Bushen Jianpi herbs on small follicle ovulation with deficiency of both kidney and spleen, and to analysis menstrual cycle, pregnancy history, thyroid function and sex hormone levels of the deficiency of both kidney and spleen type small follicle ovulation patients with hypomenorrhea.[Methods]In this study, we collected 42 cases of small follicle ovulation patients who were in accordance with both the hypomenorrhea, and the deficiency of kidney and spleen, then collected the basic information of two cases, chief complaint, present history, history of menstruation, pregnancy history, basal hormones and thyroid function, and filled in the CRF forms. Using the method of before and after own control, to observe the evaluation of symptom score, ultrasound monitoring of follicular development, endometrial situation menstrual volume and the pregnancy outcome by use of Bushen Jianpi herbs on stage treatment. Statistical analysis was performed using SPSS20.0.[Results](1) Treatment of Bushen Jianpi stage can improve the symptom of hypomenorrhea and the total effective rate was 92.5%.(2) There were significant differences in the average diameter of follicles, endometrial thickness, and endometrial ultrasound score between before and after treatment (P<0.05).(3) The curative effect of traditional Chinese medicine(TCM) syndrome score before and after treatment:the total effective rate was 97.5%. TCM symptoms before and after treatment:menstrual cycle in advance has no significant difference(P>0.05); there are significant differences in symptoms of the rest of the group (P<0.001), which shows that after the treatment of traditional Chinese medicine symptoms have improved significantly.(4) In 40 cases of small follicle ovulation, there are 9 cases of pregnancy, the clinical pregnancy rate was 22.5%. [Research conclusion](1) The therapy of invigorating kidney and strengthening spleen on staging treatment small follicle ovulation kidney and spleen deficiency syndrome the curative effect is obvious, which shows Bushen Jianpi herbs can be significantly improved in patients with kidney and spleen deficiency syndrome, increased menstrual quantity, the improvement in the menstrual cycle and improve the pregnancy rate.(2) Treatment of Bushen Jianpi stage can promote follicular development, improve the quality of oocyte, increase the thickness of the endometrium, improve the endometrial receptivity.(3) The incidence of serum thyroid stimulating hormone TSH> 2.5mIU/L was 52.5% in the patients with small follicle ovulation.
