| ObjectiveBy selecting and soring relevant factors of embryonic arrest including symptom, Tongue and pulse, find the specific performance law of embryonic arrest and the data of four diagnostic of Traditional Chinese Medicine in patients with embryonic arrest. The objective of this research is to lay the foundation for further study of embryonic arrest and provide some ideas for clinical diagnosis treatment and prevention. Analysis the TCM etiology and pathogenesis of the symptoms, tongue and pulse.MethodCollecting the related factors information of patients with embryonic arrest by questionnaire, then selecting and soring relevant factors of embryonic arrest in order to find the data of four diagnostic of Traditional Chinese Medicine in patients with embryonic arrest.First, design’the questionnaire of relevant factors of embryonic arrest’on the basis of access to relevant literature, scales and consulting the experts. Then, conduct a small-scale survey through self-designed’the questionnaire of relevant factors of embryonic arrest’. Selecting relevant factors of embryonic arrest including symptom, Tongue and pulse through the questionnaire. Entry the information collected by the questionnaire into the database of embryonic arrest established by using EpiData. At last, import the data into SPSS20.0, and statistical analyze.Results90 cases of questionnaires were distributed, and 88 cases ware recovered.63 cases were recovered at TCM clinic in Beijing Maternity Hospital and 25 cases were recovered at Guo Yi Tang clinic affiliated by BUCM. The average age of respondents is 31.64±4.08 year old and the range of the age is 25-40 year old.65 cases, accounting for 73.9% are educated in college or university,14 cases(15.9%) have a master’s degree at least,7 cases(8%) are graduated from high school,2 case(2.3%) are graduated from junior high school or below. The occupation of 24 respondents (27.3%) is the staff in government agency or company,20 cases(22.7%)are mental,11 cases(12.5%)are commerce or services,11 cases(12.5%) are full-time housewives, other occupation account for 10.2%(9 cases),7 cases (8%)are freelancer,3 cases(3.4%) are manual workers, and the occupation of 2 cases(2.3%) contact with chemicals.21.6% of respondents are considered work with a lot of pressure,54.5% are with some.45% of respondents are the only child in the family,54% are not. The average number of embryonic arrest is 1.85±0.95 and embryonic arrest happen in the range of 1-5 times.36 cases, accounting for 40.9% have 1 time of embryonic arrest,38 cases(43.2%) have 2 times of embryonic arrest,7 cases(8%) have 3 times of embryonic arrest,5 cases(5.7%) have 4 times of embryonic arrest, and 2 case have 5 times. Embryonic arrest of the respondents on average appear at 8.58 ± 2.00 weeks, and the range of the appearance of embryonic arrest is 5-14 weeks.34 cases (38.6%) of embryonic arrest appear at 8 weeks, and 12 cases(13.6%) appear at 9 weeks.10 cases (11.4%) of those surveyed fathered a child, the remaining of the respondents did not have children.88 cases of those surveyed had an average of 2.74 ±1.61 times pregnancy, in the range of 1 to 9 times.97.7% of respondents had an abortion, and the average number of abortion was 2.17 ± 1.12 in the range of 1 to 6 times.13.6% had a medical abortion and 17% had biochemical pregnancy. Contrast the mood before and after pregnancy, unpleasant rose from 8% to 19.3%, the general mood fell by 60% to 52.3%, and the happy mood decreased from 31.8% to 28.4%. The pre-pregnancy health status of 88 cases are symptoms which are the three most frequently occurring in pre-pregnancy health status. The menarche age of respondents is 13.42 ± 1.65 years old, range in 10-19 year old. There are no specific performance in the cycle of menstrual, menstrual flow, the number of days the line, texture, etc. But there are 56.8% of the dark red color of menstruation,68.2% of the menstrual with blood clots, and 51.1% of respondents with dysmenorrhea. The severe symptoms which occur when menstruation are feelings of irritability (25%), breast tenderness (22.7%) and pain of waist (21.6%). Vaginal discharge of respondents is normal.23.9% of the color is yellow in the color of vaginal discharge and there have 11.4% of the vaginal with odor.16 cases (18.2%) had genitourinary tract infections history, such as urinary tract infections and pelvic infection. There were 13 cases conducted therapy among 16 cases with genitourinary tract infections history. There were 13 cases (14.77%) with uterine fibroids, and 2 of them conducted therapy.12 cases (13.6%) had the history of surgery in the abdomen, uterus and he perineum. There were 35 cases (39.8%) with passive smoking, while there were 11 cases (12.5%) having habits of drinking.33 cases (37.5%) is like spicy,28.4% prefer a light diet,21.6% like sweets,20.5% like salty in food preferences.18 cases (20.5%) of respondents have pets, including 15 cases of dogs,cats or rabbits. There were 37.5% dyed or permed hair in a year before pregnancy, while 33% lived in the house or the company with decoration.22.7% have the habit of drinking coffee while 28.5% prefer tea.6.4% have exercise habits, of which 50% of the exercise in the form of running or jumping rope.35 cases accounting for 39.8% use a computer or watch TV every day, more than 8 hours or 4-8 hours, while 18 cases accounting for 20.5% are less than 4 hours. There are 49 cases accounting for 55.7% using the phone 4-8 hours every day,26 cases (29.5%) using the phone more than 8 hours, and 13 cases (14.8%) using the phone less than 4 hours.47 cases which accounting for 53.4% have morning sickness during pregnancy.20.4% had a cold,14.8% had diarrhea during pregnancy. In pregnancy emotions 81.8% are worried,71.6% are tense and 67% had anxiety. Lassitude (30.7%), addicted to lying (28.4%), vomiting (23.9%) and upset (18.2%) are symptoms which often appear during pregnancy. During the pregnancy, stool and urine did not change significantly compared to pre-pregnancy. Compared with pre-pregnancy,45.5% had the same diet during pregnancy, as 29.5% decreased appetite and 25% increased appetite.31.8% change in the taste during pregnancy.33% increase in sleep while 17% had a reduction. In the present situation of the respondents filling out the questionnaires, there were 50% of the pale white,29.5% pale red or zygomatic red, and 20.5% pale complexion.42% had cheek dark spots, while 25% had facial acne. About the lip color, there was 24.1% pink,33% rosy,23.9% dark red and 6.8% pale. In terms of tongue color, there was 55.7% of pink,29.5% red,14.8% pale. In terms of the shape of the tongue,85.2% was delicate,65.9% was thin,46.6% existed prick, 21.6% had teeth marks and 13.6% was sublingual vascular stasis. From the color of the fur, 73.9% is white,26.1% is yellow.96.6% of the fur was thin, while 25% was sticky.69 cases is about the drifting of pulse, including 10 cases of floating pulse and 59 cases of deep pulse.5 cases is slow while 25 cases is rapid form the speed of pulse. From the pulse intensity,13 cases was weak, while 2 cases was strong.63 cases was slippery pulse, and 1 case was unsmooth in the term of smooth degree of pulse.24 cases was like chord and 30 case was like line.Conclusions1. Surveyed patients who had a history of embryonic arrest with older age generally had a high level of education and work pressure.2. Embryonic arrest which occurred on 8.58 ± 2.00 weeks after pregnancy ranging from 5 to 14 weeks was easily repeated. More than half of embryonic arrest occurred on 8 weeks or 9 weeks and embryonic arrest had Low probability of occurrence after 12 weeks.3. Repeated abortions may be related to the occurrence of embryonic arrest. Patients with a history of biochemical pregnancy prone embryo stops growing.4.The three most appearance of physical symptoms which respondents had before pregnancy within one year was cold of the body or four limbs, cold of lower abdomen, and pain or discomfort of waist.5. Menses and vaginal discharge of respondents is normal, but more than half of them had dark color of menstruation, blood clots and dysmenorrhea. The three most appearance of symptoms which respondents had with menstruation were Feelings of irritability, breast tenderness, and pain of waist.6. More than one-third of the surveyed patients had a contact with chemicals, such as dyeing or perming one year before pregnancy or living in the renovated house one year before pregnancy. Respondents used electronic products such as computer or telephone for a long time every day.7. Most respondents have emotions of worried, nervous or anxious during pregnancy. The most four appearance of symptoms during pregnancy were lassitude, addicted to lying, vomiting and upset.8. Most of the observation of face and tongue in the present situation of the respondents filling out the questionnaires were health or in a shortage of qi and blood. The concrete performance of the shortage of qi and blood was pale or sallow complexion, pink or pale lip, delicate or thin tongue, white fur and thin fur. Of course, there are a certain percentage of heat syndrome or Yin deficiency heat syndrome. The concrete performance was pale red or zygomatic red, dark red lip, dark red tongue, tongue with prick, and yellow fur. The three most frequent pulse were the slippery pulse, the deep pulse and the pulse which is like line.9. Put forward hypothesis diagnostic information of TCM. That patients with embryonic arrest have symptoms of cold of the body or four limbs, cold of lower abdomen and pain or discomfort of waist, or with the dark red color of menstruation, the menstrual with blood clots, dysmenorrhea before pregnancy,. The severe symptoms which occur when menstruation are feelings of irritability, breast tenderness and pain of waist. A few of patients have yellow vaginal discharge, or with peculiar smell. During pregnancy lassitude, addicted to lying, vomiting and upset may appear. When filling out the questionnaire immediately, most are pale or sallow complexion, few are red face or zygomatic red. About the lip color, there was more pink, or some dark red. Some have facial dark spots, acne, or dark circles of eyes. The color of tongue are pink or red, the shape of the tongue delicate or thin, there may have prick, teeth marks, or sublingual vascular stasis. The fur is white and thin, or yellow, or sticky. There is deep pulse, slippery pulse, pulse like line, or pulse like chord. According to TCM theory, presume the syndromes as deficiency of qi and blood, deficiency of Yin and excessive of Yang, deficiency of spleen and kidney, stasis of qi and blood. |
