Objects● Aims are observing and evaluating the clinical curative effects and safety of Clinical Research of partial penetration needling on Treating Chronic localized Eczema,thus providing an effective acupuncture treatment conducive to the promotion.. Methods● Separate 60 selected patients randomly and averagely into two groups, the partial penetration needling group and the partial encircling needling group, and the course of treatment is four weeks.Therapeutic schedules:● Observation group (the penetration needling group)Acupoints:local lesions, ashi points, acupoints of fourteen meridians around local lesions,the Eczema of head and neck choose Fengchi,Baihui,Dazhui; the Eczema of arms choose Hegu,Quchi,Neiguan; the Eczema of legs choose Zusanli,Sanyinjiao,Taichong, or Xuehai Fengshi;the Eczema of back choose Pishu,Ganshu,Geshu; the Eczema of Abdmen-thorax choose Danzhong,Zhongwan;Operations:after sterilization, inserting the needles at the side of local lesions, ashi points edge, needle point toward the lesion area center with 15 ° along the skin, the needle through the lesion center for degrees, every 2cm on the edge of the lesion insert 1 needle, skin lesions diameter under 5 cm insert 1～3 needles, side by side 1 needle lesion diameter increase per 2 cm. needling selected acupoints with filiform needles and lifting, thrusting and twirling equably after getting the needling response. Courses:Four weeks, and three times a week of treating.● Control group(the partial encircling needling group) Acupoints:the same as observation group. Operations:after sterilization,insert needles around the local lesions,from the edge to the center, with 15° along the skin, to Pierce the skin of 0.5 to 1 inch, every 2cm on the edge of the lesion insert 1 needle.the operrations to last acupoints are the same as observation group. Courses:Four weeks, and three times a week of treating.● Observation targets and time-points. EASI Score:Before treatment, after the three treatment of the first week and after the second week, after the third week and the fourth week,after the sixth week,after the eighth weekend.VAS Score:Before treatment, after the three treatment of the first week and after the second week,after the third week and the fourth week,after the sixth week,after the eighth weekend.EPQOLS score:Before treatment, after the first week and after the second week, after the third week and the fourth week,● Treatment effect estimation The main evaluation index:The change of E AS I scores and VAS scores The minor evaluation index:The changes of EPQOLS scoresDuring the treatmeng the change of EASI scores and VAS scores is recorded to evaluate the clinical curative effects, and the changes of EPQOLS scores the standard of evaluating efficacy; in the meantime, safety and recurrence rate after the first, the second, the third and the fourth week of treating are evaluated. After treatment, we can get know the condition of patients by follow up-visit. Results● The comparisons of gender, age, course of disease, pathogenic site, EASI score, VAS score and EPQOLS score between the two groups before treatment have no statistical significance, and the two groups have comparability.● EASI Score(1) Intra-group comparison. The comparisons between the first and fourth week after treatment with before the treatment all have statistical significance, which means the two kinds of treatment all can improve skin lesions of chronic eczema;(2) Comparison among groups. EASI scores have no statistical significance compared the first and tourth week after treatment, which indicates the two group have no difference in improving skin lesions of chronic eczema.(3) According to the EASI scores to determine curative effects, the total effective rate and the marked effectiveness and cure rate aren’t statistically significant, which shows the two group have the same effects to improve the skin lesions of chronic eczema.● VAS Score(1) Intra-group comparison. The comparisons between the first and fourth week after treatment with before the treatment all have statistical significance, which means the two kinds of treatment all can alleviate pruritus of chronic eczema;● Comparison among groups. VAS scores have statistical significance compared the first and second and tourth week after treatment, which indicates the observation group take effects faster during the first and the second week,and have a better effects in alleviating pruritus of chronic eczema.● According to the VAS scores to determine curative effects, the total effective rate and the marked effectiveness and cure rate are statistically significant, which shows the treatment group have a better effects in alleviating pruritus of chronic eczema.● EPQOLS score(1) Intra-group comparison. The comparisons between the first and fourth week after treatment with before the treatment all have statistical significance, which means the two kinds of treatment all can improve life quallity of chronic eczema, especiallyin especially physiology index of EPQOLS;(2) Comparison among groups. EPQOLS scores have no statistical significance compared the first and tourth week after treatment, which indicates the two group have no difference in improving life quallity of chronic eczema. The improvement on physiology and psychology index of EPQOLS is more remarkable than social behavior index of EPQOLS.● Safety Evaluation:The patients in treatment group haven’t shown any relative untoward effects, which indicates the therapy of acupuncture group is safety.● Recurrence rate:The recurrent rates in the second and the fourth week after treatment of the two groups all have statistical significance, which shows the treatment group is better than the control group in the recent recurrent rate.Conclusion● Penetration needling and encircling needling have definite curative effect in improve skin lesions and alleviate pruritus of chronic eczema, and the Penetration needling take effects faster and have a better effects in alleviating pruritus of chronic eczema in first two weeks.● Both of the two acupuncture method can improve life quallity of chronic eczema, especially in especially physiologyand psychology index of EPQOLS.● The acupuncture treatment group has better effect in reducing the recurrent rate of chronic eczema.● The therapy of acupuncture is safety.
