Background:Cervical spondylosis myelopathy (CSM) is one of clinical types of cervical spondylosis.It is based on ervical intervertebral disc degeneration and facet joint degeneration, by formatting vertebral margin osteophyte, compressing the spinal cord, nerve root and blood vessels of the spinal cord, causing cervical spinal canal stenosis, chronic spinal cord compression, spinal cord blood supply disorder, then resulting in spinal cord dysfunction.Global survey shows that cervical spondylosis myelopathy is the the primary cause in adults over 50 years suffering from spinal cord injury. CSM is mainly manifested as cervical spinal cord injury, and can be accompanied with neck pain, upper limb pain and numbness. X-ray and MRI examination can assist to diagnose.Since 1995, Robinsion and Smith create anterior cervical decompression and bone graft fusion (ACDF)for the first time, it has been the standard operation, duing to its simple approach, good exposure, less bleeding, thorough decompression and stability. At present, Anterior cervical intervertebral disc resection and bone graft fusion (ACDF) is considered to be the "gold standard" for the treatment of CSM.However, although the effect of Anterior cervical decompression and bone graft fusion (ACDF) is exact, there are still many complications such as Adjacent segment degeneration(ASD),physiological activity loss, secondary instability, some patients even need reoperation. Anterior cervical decompression (Anterior Cervical Corpectomy) and bone graft fusion combined with artificial intervertebral disc replacement (ADR), namely anterior cervical fusion and non fusion "mix and match" operation, is called as Hybrid Surgery. This operation mode can not only directly decompress,but also less destruction for stability, ensure the physiological activities of the spine, and prevent ASDTCM theory argues that the cervical spondylotic myelopathy (CSM) belongs to the "Flaccidity Syndrome" and" Arthralgi Syndrome". Chinese medicine, acupuncture, massage and other means of traditional Chinese medicine is effective in the treatment of mild and moderate cervical spondylotic myelopathy。Currently many doctors advocate the use of combined traditional Chinese medicine treatment to treat cervical spondylosis myelopathy.The thesis aims to analyse the short-term clinical efficacy of this two groups of patient,explore the separate advantages and disadvantages of TCM and Western medicine,and explore the advantages of integrated traditional Chinese and Western medicine in the treatment of CSM.Objective:Through contrast the short term clinical observation of TCM combined with Hybrid Surgery group with pure Hybrid Surgery operation group in the treatment of cervical spondylotic myelopathy,explore the advantages and existing problems of Combination of traditional Chinese and Western Medicine for treating of cervical spondylotic myelopathyMethods:36 patients suffering from CSM are tread from September 2010 to October 2015。 Among these cases,17 of them are treated by traditional Chinese medicine combined with Hybrid Surgery and 19 cases are treated by pure Hybrid Surgery. Japanese Orthopaedic Association Scores (JOA), neck disability index score (NDI), Visual Analogue Scale/Score(VAS), Neural function Odom standard, X-ray index of activity of cervical spine, are performed before and,1,3,6,12 months after surgery both in two groups. Observe and analyze the curative effect.Results:Two groups of surgeries are successfully completed, all wounds healed by first intention.There is no perioperative blood vessels, nerve injury, dural tear, incision hematoma infection and any other complications. The patient’s spinal cord and neurological symptoms improved obviously, and the sensation and muscle strength are improved in different degree.From the data processing results, it can be seen that Traditional Chinese medicine combined with Hybrid operation group is better than pure Hybrid group in cervical vertebra activity degree, JO A, Odom curative effect, but there is no significant difference in Statistics (p>0.05),Chinese and Western medicine group was higher than that of simple operation group in terms of NDI and VAS with statistical differ (p<0.05).Conclusion:TCM combined with Hybrid Surgery (combination of Chinese and Western Medicine) is better than the pure Hybrid Surgery in the treatment of cervical spondylotic myelopathy(CSM).It can not only improve postoperative activity but also improve neurological function and spinal cord symptoms more better.
