Objective:Based on "the Technical Intervention Guide to the elderly fall," "The Clinical Practice Guidelines to the prevention of inpatient fall", this paper draws up the KAP scale which is fit for the elderly hospitalized patients to participate in the prevention of falls. This paper further investigates the status of KAP of elder people participating in fall prevention in order to identify the patient’s willingness to actively participate in the management or conduct of the security, therefore strengthening targeted the prevention education of fall and preventing effectively the occurrence of falls of elderly hospitalized patients.Methods:The structure of the scale based on the frame of patient participating in patient safety and the KAP mode, to form the original 43 items pool. Two rounds of expert enquiry, and pre-investigation with 35 elderly hospitalized patients lead ultimately to the formation of an initial questionnaire with 42 items. Further using convenience sampling method to select 341 cases of elderly hospitalized patients in Beijing Geriatric Hospital as respondents and to test the reliability and validity. With the methods of discrete degree, discrimination analysis, correlation analysis and exploratory factor analysis make an item analysis of the "the KAP scale for the elderly hospitalized patients to participate in the prevention of falls "; evaluate the reliability of scale with the internal consistency reliability, split-half reliability. Mainly use face validity, expert validity and construct validity for the validity index.Results:1. Construction of the initial items pool:based on "The Technical Intervention Guide to the elderly fall", "The Clinical Practice Guidelines to the Prevention of Inpatient’s Fall" of Chinese Ministry of Health and falls prevention related documents, we constituted the initial items scale. The basic constructure consists of three aspects:knowledges, attitudes and behaviors. Based on fall risk assessment, falls prevention interventions and treatment after a fall, the questionnaire with 43 initial items including patient’s general information, status of life, ability of self-care, drug factors and the specific fall prevention measures.2.Item Selection:Two rounds of expert enquiry for this study, questionnaire items after increase or decrease in the original article 43 to 42. By using the,discrete degree method entries≥0.75 standard deviation shall be maintained, items 22,24,25,26 which are less than 0.75 shall be deleted; by using discrimination analysis items 3,5,25,27,30 of no statistical significant difference (p> 0.05) shall be deleted; entries 27,30,31 screened by exploratory factor analysis shall be deleted. Items 3,5,22,24,25,26,27,30,31 and other nine items are deleted, the remaining 33 items are maintained., which constitute three factors. They were named as "participation in fall prevention knowledge", "participation in fall prevention attitude" and "participation in fall prevention act", which are completely consistent with the original building framework.3.Reliability test:The Cronbach’s a coefficient of this study questionnaire is 0.952, Cronbach’s a coefficient of knowledge, attitudes and behavior is 0.928,0.918,0.927. respectively; the split-half reliability of knowledge, attitude and behavior of this questionnaire is 0.889,0.894,0.897, which all are statistically significant (P<0.05).4.Validity test:Experts assess the content validity of 0.945, expert familiarity of 0.945, and authority coefficient of 0.945.Conclusion:1."The KAP scale for the elderly hospitalized patients to participate in the prevention of falls", which is drew up by this study, contains 33 items in three dimensions, of which 11 items of knowledge,9 items of attitudes and 13 items of behaviors.2."The KAP scale for the elderly hospitalized patients to participate in the prevention of falls" has good reliability and validity, and can be used as KAP tool for elderly hospitalized patients to participate in fall prevention, which is suitable for elderly hospitalized patients.
