Infectious wounds, belong to the category of "ulcers"in traditional Chinese medicine. In contemporary medicine, ulcers is defined as:suppurative diseases of the body surface caused by a variety of pathogenic factors in violation of the human body. External injury factors and internal injury factors affect normal skin tissues caused the wound, which is often accompanied by damaged skin integrity and a certain amount of normal tissue loss. Based on the depth of the wound, it can be divided into 3 types:type Ⅰ (Epidermal damage), type Ⅱ (Dermal damage) and type Ⅲ (Full-thickness damage). Wound healing can be roughly divided into four stages:hemostasis stage, inflammatory reaction stage, proliferative stage and remodeling stage. Factors that affect wound healing include systemic factors (including nutrition, systemic diseases, drug use, etc) and local factors (including infection, dressing change, hematoma, foreign body, etc). Infectious wounds have the clinical manifestations of local swelling, pain, skin temperature increased, pus or blood tinged fluid exudation, and even the formation of sinus or abscess. Laboratory examination shows that the white blood cell count is increased, and the exudates are cultured to see the pathogenic bacteria. According to the generation of physicians documented, the external therapy of TCM has a long-standing and various forms of not sticking to formalities. We use the disinfection dressing after the external application of Chinese medicine and Western medicine in treatment, significant effect. Ulcer oil for Beijing University of Chinese Medicine Affiliated East Dongzhimen Hospital Preparation, choose rhubarb, angelica, Ligusticum wallichii according to proportion after smashing join balm modulation and, has the effects of clearing away heat and toxic material, promoting granulation and wound healing, nourishing the skin effect, can effectively alleviate the pain of wound, improve local microcirculation, and maintain a moist environment, promote wound healing. Clinical application of simple operation, easy popularization.Objective:Observed ulcer oil external treatment with western medicine routine iodophor dressing change in the treatment of infected wounds clinical efficacy differences and to explore its possible mechanism, in order to facilitate the clinical practice of TCM.Materials and methods:From November 2015 to February 2016,70 patients with acute infectious wound in department of general surgery of Dongzhimen Ilospital were randomly divided into observation group 35 cases, control group 35 cases. After the convational iodophor disinfection, observation group with ulcer oil infiltration sterile gauze application, control group with physiological saline infiltration sterile gauze compress. Dressing was changed every other day and the observation period for 14 days. Record with the duration of disease, wound size, skin temperature, VAS pain scores and pain disappear time, skin restore normal time and wound healing time and other related data. Contrast within the group and between group differences.Results:Observation group cure rate was 48.58%, total effective rate was 100%, the control group cure rate was 17.14%, total effective rate was 94.29%, the observation group is better than that of control group, the difference is statistically significant(P< 0.05). At the same time, the observation group average skin temperature returned to normal time, and the average time of pain disappeared significantly less than the control group, the difference is statistically significant(P< 0.05).Conclusion:On the basis of routine dressing and application of ulcer oil topical treatment of wound infection, can improve the cure rate, effectively shorten the healing time, relieve pain, no obvious toxic and side effects and compliance of patients with good, worthy of clinical application. At the same time, it can further study the scope of application of ulcer oil, so that it can be more widely used in the treatment of surgical diseases.
