Until recently research on cognitive function in psychiatric disorders focused primarily on schizophrenia and unipolar depression (UPD), despite Bipolar disorder (BPD) is among the 30 leading causes of global burden of disease. One possible explanation for this is that many professionals are still influenced by the views of Kraepelin (1913) who proposed that BPD isn't a deteriorative disease, the patients with BPD can regain premorbid psychosocial function after clinical symptomatology disappears. However, it is estimated that 30%-50% of largely remitted patients fail to attain premorbid levels of psychosocial function and about a quarter have a trend to chronic deterioration. Abroad some studies have suggested that the acutely ill patients with BPD showed dysfunction in several cognitive areas, such as attention, memory and execution function, etc. Some of these dysfunction may persist in remission state. Poor psychosocial function of remitted BPD patients may be linked to cognitive impairment. However, it is difficult to draw conclusions from most of these studies due to methodologic problems.
