Objective: Cerebrovascular disease is a common diseases and a serious threat to human health. Intracerebral hemorrhage has the highest mortality of all cerebrovascular disease, the most common cause is hypertension. Since the last century from the beginning, the human beings try their best to explore surgical treatment of hypertensive cerebral hemorrhage.Now the most widely used is puncturing drainage for treatment of hypertensive cerebral hemorrhage. Using the meta analysis to analyse those high-quality published literature for comparison puncturing drainage of hematoma to conventional conservative treatment, while making a narrative of the domestic present situation of the surgery to provide a reference for the clinical neurological physicians when facing hypertensive cerebral hemorrhage.Method: We collect all published clinical research which comparing puncturing drainage of hematoma to conventional conservative treatment. To assess the quality of original studies according the requirements which is found and to analyse all researched results. We calculate the odd ratio between the puncturing drainage group and conservative treatment group to evaluate the therapeutic effects.We analyse the non-randomized case-control studies and case series studies ,then describe the present situation of the operation in China.Results:Using the META analysis to analyse seven randomized controlled clinical study, OR combined = 0.47, its 95% CI is (0.33,0.69). And then we describe the non-randomized case-control studies and observational studies case.Conclusions: The operation can lower the mortality compared to the patient who use the traditional medical treatment for hypertensive intracerebral hemorrhage.For non-randomized case-control studies and observational studies of patients,we can see that almost all the researchs shows that the surgery can reduce mortality.Puncturing drainage for treatment of hypertensive cerebral hemorrhage in China has conducted extensively, but the details of operation of all hospital has not unificated,and surgical advantages and disadvantages has both existed. For the surgery, these were not enough multi-center, large sample studies and systematic reviews of these methods. Most of all were non-randomized case-control studies and case series studies.
