Clinical Analyseis Of Elder Hyperthyroidism

Objective: Discuss the clinical manifestation, thyroid hormon, blood glucose and serum lipid of elder hyperthyroidism to find the clinical features to make the better diagnose and the therapy. Methods: Select the 146 hyperthyroidism patients which were hospitalized in our hospital from January 2005 to December 2008 were randornly. distributed to 2 groups on the base of age:elder group (age≥60, n=56) and non-elder group (age<60, n=90). Perform the retrospective analyses on patient archive, compare the clinical manifestation, thyroid hormone, blood glucose and serum lipids. Results:①Clinical manifestation of non-elder group were typic than elder group, differences has statistical significance (x~2=36.12, P=0.001). Thyroid nodules, atrial fibrillation, cardiac arrhythmia and heart failure rate of elder group is higher than non-elder group. Diffuse enlargement of thyroid and proptrusion rate evidencely lower than non-elder group, differences has statistical significance (x~2=41.67, P=0.001).②Compare the thyroid hormone, blood glucose, serum lipid of 2 groups: TT3, TT4, FT3, FT4 levels of elder group are lower than non-elder group. TSH, FPG, TC, TG, LDL-C, Apo-B, Apo-a levels of elder group are higher than non-elder group, differences has statistical significance (P<0.05). Conclusion:①Clinical manifestation of elder hyperthyroidism is not typical, thyroid nodules, atrial fibrillation, cardiac arrhythmia and heart failure rate is high. Diffuse enlargement of thyroid, protrusion rate is low.②The levels of TT3, TT4 of elder hyperthyroidism patients are not sensitivity than FT3, FT4, TSH is evidence higher than non-elder hyperthyroidism patients.③Elder hyperthyroidism is tend to disorders of glucose and lipid metabolism, and the rate is higher than non-elder hyperthyroidism patients. That made the elder hyperthyroidism misdiagnosed or leaked easily.
