Goal:Through the comparison between conservative treatment and surgery of the degenerative lumbar instability cases.the review contrastive analysis conservative treatment and the surgery treatment result and discusses degenerative lumbar spine instability methods of treatment, Exploring the use of safe and effective methods to deal with, enhances the whole treatment result.Materials and methods:Based on Linguo zhang's and CMA's diagnostic criteria and inclusion criteria, toally 67 cases were randomly divided into two groups,the conservative group(with waist protection, bed rest, bone-setting 3 on the governance, functional training, etc.) 35 cases, the surgery group(using lumbar vertebra rear guard reduced pressure+cage to plant in bone pexia) 32 cases,after treating data utilization statistics analysis contrast.Results:The surgery group was good in the first 3 months 87.5% cases were satisfied comparing the 45.71% of the conservative cases,while the percentage of satisfactory rate were closer beween the two groups,As the time lasting,after 3 months 90.63% to 60.00%; after 6 months,96.88% to 68.57%; after 12 months,93.75% to 85.71%;after 2 years,93.75% to 80.00%.So actually after 1 years (P>0.05),there is no significant difference between two groups in statistics. Two cases of conservative treatment failed, to consider further treatment; surgical treatment group cerebrospinal fluid leakage occurred in 2 cases, psychotic symptoms in 6 cases, symptomatic treatment to be cured.1 case to consider occurs close to the degeneration of motor unit temporarily to be conservative treatment.Conclusion:Surgical treatment should be carefully selected, conservative treatment is compulsory and useful.It should be considered as a first choice,no matter what kind of treatment methods should be combined spinal anatomy and biomechanics of special characteristics of the patient's sex, age, work, mental factors and living habits into account to develop a comprehensive program of personalized treatment.
