Background Ventilator associated pneumonia is the most common nosocomial infection. Mortality rates, morbidity, and costs are all increased in the patient with VAP, and every measure should thus be taken to prevent its development.Objective to investigate the incidence of ventilator associated pneumonia (VAP) in ICUs of academic hospital in China. Method The multicenter survey was carried out from June 11 to July 30, 2009. Patients aged from 10 to 80 years and with mechanical ventilation (MV) duration over 48 hours were recruited. A case report form (CRF) was used to collect clinical data. Results A total number of 314 patients were recruited in this survey. The incidence of VAP is 49.81%, and the mortality is 33.44%. A statistical significance in the incidence of VAP was found among different MV duration,categoried as≤3d,4-7d and﹥7d, (13.16%,36.43%,62.39% respectively). Meanwhile, significant changes were found in ICU length of stay, MV duration, mean angle of bed-head elevation, mortality within ICU and mean acute physiology and chronic health evaluationⅡ(APACHEⅡ) score between VAP patients and non-VAP patients. Furthermore, we compared the two group patients who were taken in ICU before and after July 10, statistical differences were found in incidence of VAP and mean angle of bed-head elevation. Conclusions This survey found a high incidence of VAP in ICUs of academic hospital in China, and it kept increasing while the ventilation days was prolonged. VAP increases ICU length of stay, MV duration and mortality . Head of bed elevation can prevent VAP effectively.
