Objective: The aim of this study is to examine the expression of TLR4 in the peripheral blood monouclear cells of VAP, and explore their association with the occurrence and prognosis of VAP. In order to explore their association with such diseases.Methods: we chose 22 patients with ventilator associated pneumonia as disease group。Analysis on disease pattern and pathogen distribution in VAP and observed the occurrence and prognosis of VAP. According to the occurrence of VAP 22 patients were divided into Early- and Late-Onset Ventilator-Associated Pneumonia group. Then according to the prognosis we divided 22 patients into poor prognosis group and better group. By flow cytometry we analyzed the expression of TLR4 in peripheral blood mononuclear cells among each group after treatment by ventilator group.Results:1. The expression of TLR4 in VAP patients was significantly higher than healthy controls (P<0.05).2. The expression of TLR4 had no difference between Early- and Late-Onset Ventilator-Associated Pneumonia group (P>0.05).3. The expression of TLR4 in poor prognosis group was significantly higher than better group (P<0.05).Conclusion:1. The expression of TLR4 in VAP patients was significantly higher than healthy controls. The result indicated that it was the Gram-negative bacteria co-infection, LPS attack, acute lung injury, mechanical ventilation may lead to increase the TLR4 expression in VAP patients.2. The expression of TLR4 had no difference between Early- and Late-Onset Ventilator-Associated Pneumonia group(P>0.05). The result indicated that the expression of TLR4 had no correlation with the time of occurrence of VAP.3. The expression of TLR4 in poor prognosis group was significantly higher than better group. The result indicated that higher TLR4 expression prognosticated the higher probability of occurrence of MODS or death。It can be used as an indicator to determine prognosis of VAP patients...
