Object:Diabetic nephropathy (DN), also known as diabetic glomerulosclerosis, is a common microvascular complication of diabetes, and one of the leading causes of death. At present, there is no efficient treatment to prevent its progression. Once the persistent proteinuria appeared, the renal function would decrease progressively. Therefore, early prevention and treatment are important for DN patients to improve the prognosis.Methods:In this paper, we reviewed the recent development of the denomination, etiology, pathogenesis, diagnosis and treatment of DN in traditional Chinese medicine (TCM) and Western medicine respectively, and also confirmed that Yiqiyangyin-decoction was efficient to DN patients in phase III-IV through clinical study. A total of 60 patients of Diabetic nephropathy with Qi-Yin deficiency was investigated and randomizely divided into two groups receiving different treatments-common treatment (control group) and the common treatment associate with Yiqiyangyin-decoction (experimental group). The clinical manifestation in TCM and several experimental indicators such as 24h Urine protein quantization(U-TP), fasting blood glucose(FBG), post-prandial 2-hour blood glucose(PBG),creatinine clearance rate(Ccr), blood fat were observed and compared pre and post treatment.Results:The overall efficacy of the two groups were significantly different (P<0.05), with a total efficacy of 93.3% in experimental group. The experimental group had significant improvement of clinical symptoms and signs (P<0.01) and reduction of U-TP, SCr and Ccr (P<0.05) post treatment, compared with the control group.Conclusion: The results showed that: Yiqiyangyin-decoction could improve clinical symptoms and signs of DN patients in phase III-IV, correct lipid metabolic disorders, reduce the visible proteinuria, adjust the filtration function of glomerulus, and finally prevent the progression of DN. Yiqiyangyin-decoction has better efficacy and less adverse reactions than western medical treatment.
