| ObjectiveIn order to reduce the high maternal anxiety, high cesarean section rate who were born after 1980, the study try to find out the state-trait anxiety and childbirth self-efficiency level, and to analyze whether there is some correlation between Childbirth self-efficiency and anxiety; this study applys touch nursing and enhance self-efficiency nursing to the pregnant women who were born after 1980, and then compares the clinical intervention effects, finally seeks a effective intervention and provide practical basis for reducing the cesarean section rate.MethodsThis study randomly selects 127 primiparaes who were born after 1980 in Tianjin Medical University General Hospital from November,2008 to October,2009, and allocateed them randomly to three groups:the touch group(42 cases), the enhance self-efficiency group(43 cases) and the control group(42 cases). The general survey, and the STAI, the CBSEI and the VAS were applied to assess anxiety level. The control group received routine obstetric nursing, the touch group and the enhance self-efficiency group were respectively given touch nursing and the enhance self-efficiency on the basis of routine obstetric nursing.The three groups primiparaes were measured objective indicators and subjective indicators at the different time during labor. The data are analyzed using the SPSS 11.5 statistical software.Results1 The S-AI scores of primiparas who are junior high school educated, singleton, at their own expense, pregnancy weight gain than or equal to fifteen kilogram and don't understand the delivery knowledge are high.2 The CBSEI scores of primiparas who have got bachelor degree or above, or are non- singleton, with medical insurance, pregnancy weight gain less than fifteen kilo-grams, and better understand the delivery knowledge are high.3 The S-AI, T-AI of primiparas are related to OE-AL, SE-AL, OE-SS, SE-SS as a negative correlation, and the correlation coefficients are -0.806,-0.744,-0.769,-0.731, 3 The S-AI, T-AI of primiparas are related to OE-AL, SE-AL, OE-SS, SE-SS as a negative correlation, and the correlation coefficients are-0.806,-0.744,-0.769,-0.731,-0.774,-0.727,-0.754,-0.686.4 The systole blood pressure, diastolic blood pressure and heart rate of the three groups were increased and accelerated at the beginning of the active phase, and at the time of the cervix to open wide and the fetal shoulder parturition. The difference within and between the groups both have statistic meaning (P＜0.05 or P＜0.01).5 The S-AI, T-AI scores at the beginning of the active phase and in 24 hours after labor, intrapartum VAS scores were lower than the control group (P＜0.05).6 The first and the second stage of labor and total labor of enhance self-efficacy group were shorter than the control group (P＜0.01). The first stage of labor and total labor of touch group were shorter than the control group (P＜0.01); the rate of cesarean section of touch group and enhance self-efficacy group were lower than that of the control group (P＜0.05). The amount of bleeding in 2 hours after labor between the three groups have statistic meaning (P＜0.05 or P＜0.01), and the amount of bleeding of enhance self-efficacy group were lower than that of touch group (P＜0.05).Conclusions1 All primiparas were in different anxiety state after admission and the self-efficacy expectations were lower than the outcome expectations. The primiparas with high trait anxiety levels and trait anxiety levels were low self-efficacy levels.2 Touch nursing and enhance self-efficacy nursing did help stress relief, raise the tolerance ability to pain, and stage of labor.3 Touch nursing and enhance self-efficacy nursing could reduce the rate of cesarean section, reduce the amount of bleeding in 2 hours after labor, shorten the first stage of labor and the total labor. Enhance self-efficacy nursing were better than touch nursing in reducing the labor and the amount of bleeding in 2 hours after labor.
