Objective:Explore the therapeutic value and the possible mechanism of maintenance hemodialysis(MHD) patients with hypertension treatment with mannitol, though observation the influences of mannitol on MAP, MDA, AOPPand NO in MHD patients with hypertension. Methods:MHD patients with hypertension were randomly divided into two groups:the conventional hemodialysis group(n1=20), and the hemodialysis plus mannitol treatment group(n2=20). The hemodialysis plus mannitol treatment group was added to 20% mannitol intravenous injection, each dosage 0.5g/kg,2 times per week for 2 months.Before and after the test all patients were measured:1. One month of SBP and DBP before hemodialysis, and MAP were calculated; 2. Serum c oncentrations of MDA,NO and AOPP, respectively using thiobarbituric acid method, nitrate reductase method and ELISA method. Results:1. Compared with before treatment, the hemodialysis plus mannitol treatment group patients's MAP is obsviously decreased after treatment (P<0.05). Compared with before treatment, the conventional hemodialysis group patients's MAP almost has no change (P>0.05).2. Compared with before treatmen, the MDA and AOPP in serum of the hemodialysis plus mannitol treatment group patients were significantly decreased after treatment(P<0.05), at the same time, the NO in serum of the hemodialysis plus mannitol treatment group patients has significantly increased after treatment (P<0.05). Compared with before treatment, there were little differences of MDA, AOPPand NO concentration in serum of the conventional hemodialysis group patients after treatment (P>0.05). Conclusion:1. Mannitol reduced MDA and AOPP concentration in serum of MHD patients with hypertension, at the same time, mannitol increased NO concentration in serum of MHD patients with hypertension. Therefore mannitol may have prompted reduction the role of oxidative stress levels in MHD patients with hypertension.2. The effect of mannitol to decrease the pressure on MHD hypertension patients may have relationship of its anti-oxidative effect and oxidative stress intervention in MHD hypertention patients.
