| Cough variant asthma (CVA) is known as a special type of asthma. Cough is the only or the main clinical manifestations.The irritating dry cough and night or morning cough are its important characteristics. Some factors,such as catching cold, cold air, dust and lampblack can result in or aggravate coughing. If there is no timely and effective treatment, some patients may develop into typical asthma. Epidemiological data show that about 5%-6% of bronchial asthma in patients only has a few stubborn coughs before the typical asthma appear. Its pathogenesis is not entirely clear, many internal and external researchers have studied its pathogenesis from different aspects, they think it relates to some factors such as the high sensitivity of the airway, airway neural regulation, inflammation, immunology, genetics, the environment, infectious factors, etc.Bronchodilator drugs and glucocorticoid are the main treatment in western medicine.In the traditional Chinese medicine, there is no the same disease as cough variant asthma, from the occurrence of the disease, the development of clinical symptoms shown by the characteristics of analysis, many doctors think that it mostly belongs to "cough","Asthma Disease", "asthma syndrome", and other category of different diseases.Based on using syndrome differentiation theory, traditional Chinese medicine treatment has its many advantages such as the overall treatment, well efficacy, less side effects, significantly improving symptoms in patients with CVA etc.Combining the clinical experience of my tutor, we think that the basic mechanism of the children's CVA is invasion of the lung by wind-phlegm and adverse lung Qi of heat. Based on that character, we use the basic treatment of expelling wind and resolving tetany, clearing away heat from the lung and resolving phlegm, normalizing qi and downbearing counterflow. We applied Xuanfeizhike decoction to treat children with CVA, and make good result. The study sets up two teams treated respectively by Xuanfeizhike decoction (treatment group) and Magan granule (comparison group),and keeps close observation over the patients in clinic. The result indicates that the numbers of the cases that were cured, significant effective, improved, ineffective in the treatment group were separately 9(30%),12(40%),9(30%),0(0%), While 2(13.3%),3(20%),7(46.7%),3(20%) in the comparison group;The total effectiveness rate of the treatment group was 100%, while 80% in the comparison group.With chi-square test, the difference of the total effectiveness rate between two groups was statistically significant(P< 0.05),and the treatment group surpasses comparison group. Furthermore the former has advantages over the latter in the following aspects such as decreasing the frequency of cough, depressing the level of cough and adjusting systemic symptoms.The study testifies the better efficiency of Xuanfeizhike decoction in treating Cough variant asthma. At the same time, it provides primary foundation for the further clinical research on Xuanfeizhike decoction. |
