Objective: to summarize the principle of Traditional Chinese Medicine Syndrome Differentiation of chronic diarrhea, and investigate the prevalence of celiac disease in patients with chronic diarrhea in Jiangsu province through the retrospective investigation.Methods:Collecting the outpatient and inpatient cases from 2002 to 2010 in Gas-troenterology department of Jiangsu Provincial Hospital of TCM, and ruling out intestinal polyps, tumors, Ulcerative Colitis and Crohn's disease and other organic disease who fulfilled the RomeⅡcriteria of irritable bowel syndrome. Chinese medical four diagnostic information and TCM distribution of patients with chronic diarrhea were collected. Detecting serological markers of celiac diasease such as IgG anti-gliadin antibodies(AGA) and IgA tissue transglutaminase(tTG) in patients with Spleen deficiency diarrhea.Results:In 170 case of chronic diarrhea patients,115 cases was male (67.6%) and 55 cases was females(32.4%), its ratio was 2.09 vs 1; The youngest was 18 years old, the oldest was 80 years, mean age was 43.09±14.93 years; the major etiologic factors for Chronic diarrhea were improper diet(clean)(46.8%), followed by bad emotions(19.8%); Weakness of the spleen and stomach and liver depression and Qi stagnation were more popular in young patients, kidney-Yang deficiency was more common in older patients; the course of illness Spleen and stomach weakness type and liver depression and Qi stagnation type, which was less than 5 years, accounted for 67.0% and 75.5% respectively. The course of illness in kidney-Yang deficiency type, which was more than 5 years, accounted for 62.5%.97 cases(57.3%) were the spleen and stomach weakness type.49 cases(28.7%)were the liver depression and Qi stagnation type.24 case(14.0%) were kidney-Yang deficiency type. In 73 patients with Spleen deificiency diarrhea, IgG AGA were positive in 5 cases, IgA tTG were positive in 2 cases, both positive in 1 case, male to female ratio was 1:1.Conclusion:TCM distribution of chronic diarrhea in descending order is spleen and stomach weakness type> liver depression and Qi stagnation type> kidney-Yang deficiency type. The prevalence of chronic diarrhea had different characteristic in different gender, age, course and life factor. The serum antibody positive of celiac disease in Spleen deficiency diarrhea was 1/12(8.2%). Celiac disease may be one of important cause to Spleen deficiency Diarrhea. It prompts the screening of celiac disease in the future.
