Objective:To observe clinical results of cervical spondylotic myelopathy (CSM) with anterior decompression and further more,to analyze the factors affecting prognosis.Methods :52 cases of cervical spondylotic myelopathy treated by anterior operation were reviewed,8 with anterior decompression and fusion with Trepan (ADFT),15 with cage,25 with cerviacal spinal locking plate (CSLP),4 with artificial cervical disc replacement (ACDR),The Japanese Orthopedic Associaction (JOA) scoring system was used to evaluate cervical myelopathy,and the recovery rate calculated 1 week and 3 month afeter surgery. The treatment results and common influencing factors were analyzed.Results:(1) JOA scores of pre-post-operation and 3 month follow-up were 8.42±2.69,12.82±1.91,14.27±1.89 respectively, which showed a trend of significant increase.(2) The treatment results had relationship to patient's age,preoperative condition and operative way (p<0.05)。.(3) Abnormal cervical curve, intramdullary signal change found before surgery and with low canal-body ratio were poor in treatment outcome (p<0.05).(4) the treatment results had no relationship to operative segment (p>0.05)Conclusion:(1)anterior decompression is an effectively surgical treatment for CSM.(2) Patient's age and preoperative JOA score are good prognostic indicator to judge the result after anterior decompression. (3) The cervical segment stability, canal-body ratio and intramedullary sinal change are useful parameters to forecast the recovery of CSM.
