Objective: To observe clinical results of cervical spondyloticmyelopathy (CSM) with posterior decompression, and furthermore, toanalyze the factors affecting prognosis.Methods: 31 cases of cervical spondylotic myelopathy treated byposterior decompression operation were reviewed, 10 with laminectomy,10 with hemi-laminectomy,11 with open door laminoplasty(ODLP) at thesame time. The Japanese Orthopedic Association(JOA) scoring systemwas used to evaluate cervical myelopathy, and the recovery ratecalculated 3 month after surgery. The treatment results and commoninfluencing factors were analyzed.Results: (1)JOA scores of pre-, post- operations and follow—upwere 11.38±2.66, 14.51±2.36, 14.77±2.26 respectively, which showed atrend of significant increase. (2)The treatment results had relationship topatient's age, duration of symptom, preoperative condition and area ofspinal cord at the maximum compression. But the treatment results had norelationship to operative methods. (3)Abnormal cervical curve,intramedullary signal chang found before sugery were poor in treatmentoutcome.Conclusions:①posterior decompression is an effectively surgicaltreatment for CSM with cervical spinal stenosis.②Patient's age, duration of symptom, preoperative JOA score and area of spinal cord at themaximum compression are good prognostic indicator to judge the resultafter posterior decompression.③The cervical curve, segment stability,intramedullary signal change and the pre-or postoperative area of canalare useful parameters to forecast the recovery of CSM.
