Multivariate Prognostic And Recrudescent Analysis In Gastric Carcinoma Patients After Operation

Objective:To analyze the related factors affecting the prognosis and recurrence of gastric carcinoma after operation.And to provide scientific proof for raising the long-term survival rate of the patients by controlling the crisis factors.Methods:The date of 877 patients who were diagnosed by Preoperative and after operation from Jan.2001 to Dec.2005 in Tianjin cancer hospital were investigated retrospectively,and they all underwent operation of gastric carcinoma including stageⅠ~Ⅲ,88 patients were lost to be visited and 169 patients who were visited to recurrence.The Kaplan-Meier method and log-rank test were used for univariate comparisons of survival.Multivariate analysis was done by the COX regression model.Result:According to the Kaplan-Meier and log-rank methods,depth of tumor invasion,lymph node metastases, the size of tumour, diseased region,whether or not to transfer distant place,TNM stage and whether or not to give radical surgery were prognostic factors.Multivariate regression analysis indicated that only lymph node metastases, whether or not to transfer distant place,TNM stage and whether or not to give radical surgery were independent prognostic factors.There were 878 patients who were followed up to death or lost to be visited in this group,the mean survival time was 47.538±1.277 months (95%CI 45.034~50.041),the median survival time was 41.750±4.594 months (95%CI 32.746~50.754).The 1-,3-and 5-year cumulative survival rates were 81.88%(646/789),51.31%(405/789),40.38% (318/789),respectively.But according to the Kaplan-Meier and log-rank methods, that lymph node metastases and whether or not to give radical surgery was related to recurrence. Multivariate regression analysis indicated that only lymph node metastases was independent enflunced factor. The mean recurrenced time was 24.853±1.882 months (95%CI 21.164~28.542), The median recurrenced time 15.000±1.898 months (95%CI 11.281~18.719). The 0.5-,1- and 3-year cumulative recurrenced rates were 20.71%(35/169).46.75%(79/169),87.57%(148/169).Conclusion:Gastric carcinoma is a kind of tumour which is high incidence in digestive tube tumour and the factors that affect long-term existence are multitude,among of them, TNM stage is closed to long-term existence,especially, lymph node metastases.As clinic workers,controlling these risk factors actively could improve the survival rate of gastric carcinoma patients.
