Objective To study the relationship in the level of CD3-/HLA-DR+, CD3+/CD25+ and CD3+/HLA-DR-,liver fuction test and complications in patients after liver transplantation, and evaluate the significance of the level of CD3-/HLA-DR+, CD3+/CD25+and CD3+/HLA-DR- in immune status of the recipientsMethods During May 2003 to November 2009, the liver functions and lymphocyte subsets in 52 recipients who had liver transplantation in our hospital during were detected by the automatic biochemical analyzer and flow cytometry. The recipients were divided into two groups (normal and abnormal groups) according to the liver function tests (total bilirubin, direct bilirubin, indirect bilirubin, anlanine aminotransferase and aspartate aminotransferase). The recipients in the abnormal group were divided into the acute rejection group, drug reaction group and unknown reasons group according to liver biospy, plasma concentration of FK506 and symptoms in recipients with abnormal liver fuction tests.Results In normal group, the expression of CD3-/HLA-DR+ was significantly lower than that in abnormal group(14.09±9.36 and 19.81±9.11,P=0.011), the expression of CD3+/CD25+ was lower than that in abnormal group (1.97±0.79 and 2.67±0.98,P=0.002), the expression of CD3+/HLA-DR- was higher than that in abnormal group(60.54±14.79 and 52.34±12.66, P=0.012).In the acute rejection group, the expression of CD3-/HLA-DR+(25.87±9.43) and CD3+/CD25+(3.20±0.87) were higher than those in drug reaction group (16.00±7.23,P=0.039; 2.29±0.83,P=0.048), the expression of CD3+/HLA-DR-(43.04±8.10) was lower than that in drug reaction group(56.65±10.06, P=0.007).Conclusion The expression of CD3-/HLA-DR+, CD3+/CD25+ and CD3+/HLA-DR-in recipients with liver transplantation can reflect the immune status of the recipients in time, and different level of expression of CD3-/HLA-DR+, CD3+/CD25+ and CD3+/HLA-DR+in recipients has significant correlations with complications after liver transplantation, they may be uesed to assess the complications and the immune status of recipients after liver transplantation. When there are high expression of CD3-/HLA-DR+, CD3+/CD25+ and low expression of CD3+/HLA-DR- in recipients, we can predict that rejection may occur in recipients. And when there are low expression of CD3-/HLA-DR+, CD3+/CD25+ and high expression of CD3+/HLA-DR- in recipients, drug reaction may happen in recipients.
