Objective:To improve the efficiency in the diagnosis of infective endocarditis(IE) with echocardiography by analyzing the echocardio graphy characteristics and hemodynamics change of IE diagnosis cases and misdiagnosis cases.Methods:The echocardiography and hemodynamics change data of 71 patients with IE detected by echocardiography in our hospital from January 2001 to July 2008 were retrospectively analyzed and compared with operation and blood culture in order to collect the echocardiography characteristics of IE.Results:Among 71 IE patients diagonosed by echocardiography,64 cases were confirmed with IE after blood culture, operation and pathology,2 cases were misdiagnosed and 5 cases were excluded from this research for lack of after-surgery diagnosis and blood culture. In the 64 confirmed cases,57 cases were detected to have valve vegetation with transthoracic echocardiography.7 cases were detected not to have valve vegetation with transthoracic echocardiography but vegetations(size less than 3 mm) were found by TEE.50 patients among those 64 confirmed cases were under operation and the location, size of vegetation detected by echocardiography were consistent with the surgery, while 14 cases were diagnosed to be with IE after positive blood culture. With after-surgery pathology diagnosis,1 of the 2 misdiagnosed cases was rheumatism valve fibrination, while another was valve ageing pathology and valve calcification. The positive examination rate of echocardiography was 97%(64/66); the misdiagnosis rate of IE was 3.0%(2/66).Conclusion:Echocardiography has high sensitivity and specificity in detecting IE and valve vegetations, can provide valuable informations for valve complications like valve perforation or valve damage and plays an important role in diagnosing infective endocarditics and related diseases.
