Clinical Characteristics Of 95 Patients With Infective Endocarditis In Congenital Heart Disease

[Bacground]:To summarize the clinical characteristics of infective endocarditis in patients with congenital heart disease.[Methods]:Retrospectively,we reviewed all medical records of definite CHD-associated IE between January 2010 and May 2017 who were treated at Peking Union Medical College Hospital.We defined a case of CHD-associated IE through TTE and Duke criteria.[Results]:(1)Since from January 2010 to May 2017,a total of 95 patients were diagnosed with congenital heart disease with infective endocarditis(in accordance with modified Duke criteria),and the average age is(37.7 ± 13.3)y(14?66y),with male 64 cases,(37.4±13.9)y,female 31 cases,(38.4±12.0)y,and the gender ratio is 2.06.(2)Aortic valve two-lobe deformity is the most frequent CHD in CHD associated with IE,followed by ventricular septal defect.(3)Most cases of CHD-associated IE are caused by Streptococcus spp.,including Streptococcus viridans group and Enterococcus spp.(4)The sensitivity of transthoracic echocardiography is 86.3%.(5)There were 81 patients received surgery,among whom 2 patients were dead,another 14 patients underwent conservative treatment of internal medicine,1 patient dead.The overall mortality rate was 3.2%.(6)The common complications were cardiac complications(n=81),embolizations(n=21),and renal impairment(n=5).[Conclusion]:(1)Complete CHD repair may eliminate the risk for IE,therefor screening for CHD is necessary;(2)Compared with structural heart disease,CHD-associated IE is more likely to involve right heart;(3)When conservative treatment of internal medicine is less effective,early surgical treatment is recommended.
