Pre-eclampsia is a multi-system disorders occurred after 20 weeks gestation. It is characterized by an abnormal vascular response, such as systemic vascular resistance increased, platelet aggregation enhanced, coagulation system activation and endothelial cell dysfunction. Based on the clinical findings, pre-eclampsia can be viewed as a maternal syndrome (hypertension, proteinuria, etc.) can also be viewed as fetal syndrome (growth retardation, amniotic fluid decreased oxygenation abnormalities). The pathogenesis of pre-eclampsia remain unclear. The cause of the doctrine of pre-eclampsia include immune mechanism, shallow placental implantation, vascular endothelial cell dysfunction, genetic factors, etc. It now widely recognized that shallow placental implantation and blood vessel happened Recasting obstacle is the pathological basis of pre-eclampsia. And the vascular endothelial injury is a central problem. A large number of studies on cardiovascular confirmed that lox-1 and oxLDL can cause vascular endothelial cells and vascular smooth muscle cells damage and foam cells formation, that it is the independent risk incidence of cardio-cerebral vascular diseases. Many studies have found that hypertensive disorders in pregnancy, especially pre-eclampsia often accompanied by dyslipidemia. And it accompany with acute atherosclerosis at myometrial spiral arteries and the last paragraph of uterine decidual spiral arteries and extensive damage to the endothelial.The increase of oxLDL in peripheral blood can induce the senile of endothelium progenitor cells.Thereby,Which can lead to the plerosis function disability and the damnification disbalance of the endothelium. Now,Research abroad has detected LOX-1 expression in preeclampsia and normal placenta and apoptosis of sertoli cells,and has found that LOX-1 expression and setoli cells' apoptosis in preeclampsia placental syncytiotrophoblast is significantly higher than that in normal placenta tissue.But, There is no report about the expression of LOX-1 in the peripheral blood and the significant difference in the preeclampsia.Otherwise,Pre-eclampsia is a severe complication in pregnancy,which has threatened the healthy of mothers and infants, and it is the second in the maternal death rate.Above all, the early-onset preeclampsia always is the tickler which obstetricians have to face. Preeclampsia-eeclampsia is a multisystem disorder associated with hypercoagulability, fibrin deposition, and thrombosis. It may prevent and cure the early-onset preeclampsia, if we should improve hypercoagulability. Moreover, it has been reported to be associated with thrombophilia by case controls and prospective studies, such as salvia miltiorrhiza,low-molecular weight heparin.section 1 The Expression of LOX-1,oxLDL in the peripheral blood in Pre-eclampsiaObjective:study the expression of LOX-1,oxLDL in peripheral blood in Pre-eclampsiaMethods:The expression of LOX-1 is detected by RT-PCR and Western blot between 54 cases of pregnancy induced pre-eclampsia patients and 18 cases of normal pregnancy women in the blood mononuclear cells. The level of oxLDL in Serum is detected by ELISA.Results:From normal pregnant women to severe pre-eclampsia patients, the expression of LOX-1 and oxLDL in peripheral blood mononuclear cells show a rising trend. And the difference between each two groups in three groups was significance. (p<0.01)Conclusion:LOX-1, oxLDL is highly expressded in the peripheral blood of patients with pregnancy induced hypertension. And it suggested that LOX-1 could be measured pre-eclampsia's progress.section 2 Clinic study of the early-onset preeclampsia with salvia miltiorrhiza,low-molecular-weight heparin therapyObjective:TO evaluate the clinic curative effect of early-onset preeclampsia with salvia miltiorrhiza,low-molecular-weight heparin therapy.Methods:In the period from 2007.9 to 2009.10,90 early-onset preeclampsia women from the obstetric department of the Second Xiang Ya Hospital of Central South University were involved in this project. The participants were randomly divided into four groups:magnesium sulfate group (group 1,38 cases), magnesium sulfate and salvia miltiorrhiza(SM)group (group 2,28 cases), magnesium sulfate and low-molecular weight heparin (LMWH)group (group 3,12 cases) and magnesium sulfate and SM and LMWH group (group 4,12 cases). Collected the data means,delivery information of these participants,ordinary information and subjective symptom which before and after the treatment, checked and compared the blood cells count, urine protein, liver and renal function test, coagulation function test, and so on which before and after the treatment, and estimated the clinic curative effect of early-onset preeclampsia with SM,LMWH therapy.Results:(1) To improve patient subjective symptom, There were significant differences among four groups (p<0.05). magnesium sulfate plus SM group have reach 96.43%, which obviously surpass than magnesium sulfate alone group.(2)There were no significant differences among four groups in Hct,ALT,PT,APTT,UA and urine protein in 24 hours which after the treatment 24 hours (p>0.05).But magnesium sulfate plus SM,LMWH has significant differences with magnesium sulfate alone group in decrease urine protein in 24 hours and prolong activated partial thromboplastin time (p<0.05). Magnesium sulfate plus SM,LMWH group have a fine tendency to reduce hematocrit,improve liver and renal function which contrasted to the magnesium sulfate alone group.(3).There were significant differences among four groups in lengthen gestational weeks (p<0.01). magnesium sulfate plus LMWH group has the longest days,and the other two groups have more days than magnesium sulfate alone group.(4).There were differences among four groups in raise neonatal survival ratio (p<0.05).Among groups, magnesium sulfate plus LMWH group has the maxlmum ratio, the other two groups also have more ratio than magnesium sulfate alone group.Conclution:Using magnesium sulfate plus SM,LMWH could improve patient subjective symptom,prolong the pregnant time and raise neonatal survival ratio of early-onset preeclampsia, and which have significant effectiveness in decrease urine protein in 24 hours and prolong activated partial thromboplastin time. Meanwhile, Whether or not to improve liver and renal function,there was needed large samples study to ascertain the effective of these groups.
