[Objective] Through the comparison of distribution of HLA-G protein in eutopic and ectopic endometrial tissues of patients with adenomyosis (AM) and endometrial tissues of patients only with hysteromyoma,the thesis intends to investigate the role and significance of HLA-G protein in the pathogenesis of adenomyosis.[Methods] Immunohistochemistry and western-blotting methods were used for detecting the expression of HLA-G protein in eutopic and ectopic endometrial tissues of 31 patients with adenomyosis (AM),normal endometrial tissues of 18 patients only with hysteromyoma were recruited as control group. The expression of HLA-G in the pictures was analyzed by Image-Pro Plus 6.0 and JIE DA JD-801 image analysis systems and the software of SPSS 16.0 was used for analyzing the experimental data.[Results] (1) The expression of HLA-G in eutopic and ectopic endometrial tissues of patients with AM The expression of HLA-G was in glandular and stromal cells in eutopic and ectopic endometrium of patients with AM,whereas the expression of HLA-G in endometrium of patients with hysteromyoma (control group) was not detected;There was no obvious difference in the expression of HLA-G protein between eutopic and ectopic endometrium of patients with AM(P>0.05);There was no significant difference in the expression of HLA-G in eutopic endometrium of patients with AM between proliferative phase and secretory phase(P>0.05). (2) The expression of HLA-G in glandular and stromal cells of patients with AM The expression of HLA-G in glandular cell in eutopic and ectopic endometrium of patients with AM was higher than that in stromal cell of AM(P<0.01).In glandular cells, the expression of HLA-G in ectopic endometrium was higher than that in eutopic endometrium;But in stromal cells,the expression of HLA-G in ectopic endometrium was lower than that in eutopic endometrium(P<0.01).[Conclusion] The comparison between the expression of HLA-G in eutopic endometrium of patients with AM and the absence in endometrium of patients with hysteromyoma (control group)illustrated that the expressions of HLA-G between the endometrium of control group and the eutopic endometrium of patients with AM are different, and great changes have been made before the eutopic endometrium invading myometrium.There was no significant difference in the expression of HLA-G in eutopic endometrium of patients with AM between proliferative phase and secretory phase. The expression of HLA-G has not been impacted by estrogen and progestin. The higher expression of HLA-G protein in ectopic endometrium tissues could participat in occurrence and development of AM by evading body's immune surveillance. The expression of HLA-G in glandular cell in eutopic and ectopic endometrium of patients with AM was higher than that in stromal cell of AM., which showed that the expression of HLA-G was related to the tissue specificity. That the expression of HLA-G in glandular cells in ectopic endometrium was higher than that in glandular cells in eutopic endometrium makes ectopic endometrium more resistive. The study of HLA-G may provide a new thread on the treatment of adenomyosis.
