Objective:By the analysis of mortality rates of Anren county, Hunan, in years 2004-2005, studying the distribution characteristics and the burden of diseases of malignant tumors in this district in order to determine the high risk population and provide the scientifi base for the prevention and treatment of malignant tumors.Methods:The method of third national retrospective sampling survey of causes of death being adopted, the information about cancer -related deaths of residents in Anren county of Hunan is collected. statistics on the malignant tumor-related crude population mortality, standardised mortality, constitution of causes of death, the years of potential life lost(YPLL) and the related indexes are collected, and an analysis of malignant tumors-related death distribution is made.Results:1. In 2004-2005, the crude mortality in Anren county was 134.12/ 100000, standardised mortality was 98.26/100000. the crude mortality of male was 163.29/100000 and the standardised mortality was 120.50/ 100000. The crude mortality of female was 102.44/100000 and the standardised mortality was 74.03/100000. The crude mortality and the standardised mortality of male were higher than those of female (u=1789.87, P<0.01).2. The top 5 cancers in terms of high mortality were as follows:liver cancer, gastric cancer, lung cancer, leukemia and esophageal cancer. Among the top 10 cancers were mainly digestive system tumors. The mortality of male was higher than that of female (not including cervix cancer and breast cancer).3. The mortality of malignant cancers in 2004,2005 and in the two years as a whole increased along with growing age (x2=934.78, P<0.001, x2=934.78, P<0.001,x2= 2383.873, P<0.001). The mortality of various kinds of malignant cancers increased more obviously after the age of 60.4. In 2004 and 2005, the mortality of malignant tumors in mountain areas was higher than that in basins (141.97/100000 and 123.27/100000, x2=5.060, P=0.024 respectively), and that in rural areas was higher than that in urban areas.5. The top 5 cancers in terms of YPLL among both male and female were as follows:liver cancer, gastric cancer, leukemia, lung cancer and brain tumor. The top 5 among males were liver cancer, gastric cancer, lung cancer, leukemia and brain tumor; the top 5 among females were liver cancer, gastric cancer, cervical cancer, leukemia and lung cancer.6. The average years of potential life lost(AYPLL) among males(16.53) were less than those among females(17.00); the top 5 cancers in terms of AYPLL among males and females were as follows: leukemia, nasopharyngeal cancer, brain tumor, cervical cancer and liver cancer.Conclusion:1. In Anren county in 2004-2005, the general mortality of malignant tumors among males was higher than that among females, that in mountain areas was higher than that in basins, and that in rural areas was higher than that in urban areas. It increased along with growing age; there was a tendency of youthization; and the population after the age of 60 was high risk group.2. The highest mortality of malignant tumors of the County was among digestive system tumors such as liver cancer, gastric cancer, esophageal cancer. So target-oriented comprehensive work of cancer prevention and control shoud be carried out.3. The most perniciousness of malignant tumors that resulted in the life lost in the area were liver cancer, lung cancer, brain tumor, leukemia and cervical cancer. So the work of prevention and control against the above-mentioned cancers should be vigorously strengthened.
