Coronary heart disease(coronary heart disease IHD)is a kind of common disease in clinic,which have feature of high lethality. The time of anti-ischemia and anti-hypoxia is 20-40 minute in cardiac muscle cell,when the time of ischemia and hypoxia is over-30 minut, cardiac muscle cell will appearance hibernation,enve decease. Gua-Lou-Tong-Mai-Wan (It was composed of gualou, dangshen, danshen, yanhu, three-seven powder, chishao, baishao etc.) which has the function of regulating the flow of vital energy, removing phlegm, and regulating blood circulation can improve the clinical syndrome of coronary heart disease. It was made by a decoction of change after many year's clinical observation. The subject observed Gua-Lou-Tong-Mai-Wan on blood lactic dehydrogenase (lactic dehydrogenase LDH),creatin kinase(creatin kinase CK),and electrocardiagram(electrocardiagram ECG),heart rat(eheart rate HR),mean artery pressur(emean artery pressure MAP),left ventrcular end-systolic pressure ( left ventrcular end-systolic pressure LVESP ),left ventrcular end-diastolic Pressur(eleft ventrcular end-diastolic Pressure LVEDP),maximum rising rate of left ventrcular systolic pressure(maximum rising rate of left ventrcular systolic pressure +dP/dtmax),maximum declining rate of left ventrcular diastolic pressure(maximum declining rate of left ventrcular diastolic pressure -dP/dtmax)of acute myocardial ischemia(acute myocardial ischemia AMI) rats to approach the efficacy and mechanism of the new formulation on cardiovascular disease.Objective:To study and research the effect of Gua-Lou-Tong-Mai-Wan on left ventricular function(left ventricular function LVF)and myocardium architecture of AMI rats. To initial investigate a possibility of anti- myocardial ischemia and protection in heart function.Methods:Select male SD rats 50, weight from 180g to 210g. After 5 days normal forage feeding, they were divided into 5 groups(each group of 10) at random: Normal control group, Model control group, High-dose of Gua-Lou-Tong-Mai-Wan group, Low-dose of Gua-Lou-Tong-Mai-Wan group, metoprolol control group. Except for 0.9% isotonic Na chloride injection in normal group, the rest of groups were made a mode of AMI based on Rona,s method which was given 5mg·kg-1·d-1 ISO by subcutaneous injection to copy acute myocardial ischemia model in rats.At the same time, control drug(dose of 2g/kg·d) or treatment drug(dose of 2g/kg·d, 4g/kg·d respectively) was given in each treatment group,metoprolol group was stomach-perfusion once a day in 10 mg·kg-1·d-1 dose, the rest of groups was given Isodose Nacl,every groups was treated for 10 days.TO record the change of ECG and LVF in every groups,det the content of blood LDH and CK,and obverse myocardium form when lasted for 12 hours of abrosia at the end of the dose.Result:Compared with Normal control group,the level of LDH and CK was significant advancement(P<0.05),∑ST section was obvious shift(P<0.01),and transverse striation of cardiac muscle cell has disappeared,the normal cell structure has demolished,the endochylema was anachromasis in model control group,which showed that the model of AMI dupilacted successfully. Compared with model control group,The level of LDH and CK was significant descent (P<0.01 or 0.05),the High-dose of Gua-Lou-Tong-Mai-Wan group was better than Low-dose of Gua-Lou-Tong-Mai-Wan group and metoprolol group to cut down the CK level. All observed groups have no obvious disparity to lower the LDH level(P<0.01).Compared with model control group,∑ST section shift was significantly improved(P<0.05 and 0.01), Low-dose of Gua-Lou-Tong-Mai-Wan group have no obvious shift.Compared with model control group, the level of +LVdp/dtmax was signicant raise in metoprolol group and high-dose of Gua-Lou-Tong-Mai-Wan group(P<0.01). the level of LVESP was signicant raise in Low-dose of Gua-Lou-Tong-Mai-Wan group(P<0.01), the level of -LVdp/dtmax was obvious descent in all treated groups(P<0.05), metoprolol group and high-dose of Gua-Lou-Tong-Mai-Wan group were better(P<0.01).In normal control group,cardiac muscle cell transverse striation was clear, cell nucleus was normal and the stroma was not found vasodilatation and inflammation cell infiltration.In model control group, cardiac muscle cell transverse striation was defluxio,normal cellularity was destroyed, endochylema anachromasis,and focal necrosis.In low-dose of Gua-Lou-Tong-Mai-Wan group, cardiac muscle fiber reflected swelling, cardiac muscle cell transverse striation was defluxio, wide cell vacuolar degeneration, to display hyperemia and hemorrhage band in part. In high-dose of Gua-Lou-Tong-Mai-Wan group, cardiac muscle fiber cloudy swelling degeneration, cardiac muscle cell transverse striation was defluxio, large cell vacuolar degeneration, In metoprolol group, cardiac muscle fiber cloudy swelling degeneration,to display fiber break,focal necrosis and inflammation change was obvious. Predictably, the damage degree of myocardium all treated groups was alleviative to compared model control group,the high-dose of Gua-Lou-Tong-Mai-Wan group was notable.Conclusions:1 Gua-Lou-Tong-Mai-Wan can signicantly reduce leakage of LDH and CK from cardiac muscle cell,which explain Gua-Lou-Tong-Mai-Wan can protect the integrality of cardiac muscle cell membrane.2 Through to observe the ECG, hemodynamics, myocardial enzymes index, certify Gua-Lou-Tong-Mai-Wan can obviously improved∑ST section shift, to lower LVEDP and -LVdp/dtmax. to advance LVESP and +LVdp/dtmax. Confirm Gua-Lou-Tong-Mai-Wan can obviously improved rats heart function of AMI.3 Through to observe myocardium pathology,to certify Gua-Lou-Tong-Mai-Wan can decrease the area of ischemia myocardium, and agonistic myocardial infarction(myocardial infarction MI).4 The experiment obverse the effect of ECG, hemodynamics , myocardium enzymology ,and pathology by different dose Gua-Lou-Tong-Mai-Wan, confirm high-dose of Gua-Lou-Tong-Mai-Wan was the best to anti-MI and improve heart function.
