Objective Explore the rule of changes of myocardial enzyme spectrum level in patients with cerebral hemorrhage. Analysis the relation between increasing myocardial enzyme spectrum level and re-bleeding, and the effects of changes of myocardial enzyme spectrum level on prognosis .So as to provide the clinical basis for diagnosis and treatment in patients with cerebral hemorrhage accompanying with elevated myocardial enzyme spectrum level.Methods 60 clinical cases of patients with cerebral hemorrhage were selected and were divided into two groups according to these who accompanied with elevated myocardial enzyme spectrum level or not, then compared with each other, calculating dynamic the rate of cerebral hemorrhage with elevated myocardial enzyme spectrum level; Dynamic observation to these patient with cerebral hemorrhage who were enrolled after 1day, 3 days, 7 days,14 days, and 28days, Summarize cerebral hemorrhage dynamic changes of myocardial enzyme; Grouped according to grading standards consciousness,then analyzed the relationship between the degree of disturbance of consciousness and myocardial enzyme levels; Grouped according to the bleeding site, analyzed the relationship between bleeding sites and myocardial enzyme levels According to the mortality of patients with cerebral hemorrhage whether myocardial enzymes levels is elevated or not, analysise relation between myocardial enzyme levels and prognosis in patients with cerebral hemorrhage.Results Cerebral hemorrhage 1day, 3days, 7 days, 14 days, 28 days, accompanied by elevated myocardial enzyme spectrum level abnormal rate separately is 67%,54%,38%,11%,2%; Myocardial enzymes elevated in patients with re-bleeding after cerebral hemorrhage is significantly higher than that of without re-bleeding(P<0.05);Biochemical indicators of myocardial enzymes after intracerebral hemorrhage except CK-MB began to increase within 24 hours, reach the peak after 3 days , then gradually return to normal; Myocardial enzymes level in patients with disturbance of consciousness is higher than that of without disturbance of consciousness (P <0.05); Myocardial enzyme levels of cerebral hemorrhage in the brain stem, thalamus, cerebellar hemorrhage is severer than those of cerebral lobes, basal ganglia;The mortality rate of patients with cerebral hemorrhage accompanied by elevated myocardial enzyme spectrum level is higher than that of patients with normal myocardial enzyme spectrum level.Conclusions Cerebral hemorrhage associated with elevated myocardial enzymes level, the abnormal rate of secondary myocardial damage is higher; Cerebral hemorrhage increased myocardial enzyme levels have a dynamic process of change, 3 days peak, then gradually return to normal; Cerebral hemorrhage associated with significantly elevated myocardial enzymes level in patients have increased likelihood of re-bleeding; Myocardial enzymes level in patients with disturbance of consciousness is higher than that of without disturbance of consciousness;Myocardial enzyme levels after cerebral hemorrhage is a reference to assess prognosis of patients who suffered cerebral hemorrhage. Dynamic monitoring of cerebral hemorrhage in myocardial enzyme changes, especially a disturbance of consciousness, bleeding sites in the hypothalamus, brain stem and cerebellum sites help early detection of myocardial damage in patients with primary disease and the aggravation, to cerebral hemorrhage with myocardial damage in patients with brain-heart syndrome provide a theoretical basis on the diagnosis and treatment for clinicians. When treat cerebral hemorrhage, clinicians should monitor the abnormality of the heart,protect the heart,identify the severity of primary disease and improve prognosis.
