Effect Of Sapylin By Preoperative Intraperitoneal Injection On Advanced Gastric Cancer

Objective Sapylin is one of the biological response modifiers. As one of the comprehensive treatment methods for advanced cancer, the clinical efficacy of sapylin on advanced cancer has been proved. This study was to evaluate the effect of preoperative intra-peritoneal injection of sapylin and intraoperative peritoneal ehemo-hyperthermia on advanced gastric cancer.Methods Seventy-nine patients expected radical gastrectomy were randomly divided into the treatment group(sapylin+mitomycin c, 40 patients) and the control group(mitomycin c alone, 39 patients).In the treatment group, 5 KE sapylin was intraperitoneally injected into peritoneal cavity 48 h before operation and patients need to receive peritoneal ehemo-hyperthermia when in operation and then 4 mg of mitomycin c was injected into peritoneal cavity before the closure of the peritoneum. In the control group, patients only need to receive peritoneal ehemo-hyperthermia when in operation and then 4 mg of mitomycin c was injected into peritoneal cavity before the closure of the peritoneum.Results Two patients(5.0%)died of peritoneal metastasis in treatment group and nine patients(23.1%)died of peritoneal metastasis in control group. There is a statistical difference between the two groups on the rate of patients who died of peritoneal metastasis (P<0.05). Peritoneal metastasis occurred in treatment group for 6 cases(15.0%)and the time of occurrence of peritoneal metastasis on average is(15.50±6.47)month. Peritoneal metastasis occurred in control group for 14 cases(35.9%)and the time of occurrence of peritoneal metastasis on average is(9.29±4.41)month. There is a statistical difference between the two groups on the rate and time of peritoneal metastasis (P<0.05). The difference between the two groups on exhaust time, defecation time and hospitalization time showed no statistical sense. Postoperative complications showed no significant difference between the two groups (P>0.05). There is a significant difference between the two groups on Spitzer and Chew-wun Wu QOL scores after operation. The difference of The 3-year survival rate was significantly higher in the treatment group than in the control group (76.5% vs 49.4%, P<0.05).Conclusions Preoperative intra-peritoneal injection of sapylin can increase the 3-year survival rate after radical gastrectomy, decrease the rate of peritoneal metastasis and death by peritoneal metastasis, delay the occurrence of peritoneal metastasis and improve postoperative quality of life of patients but do not increase the incidence rate of operative complications and do not affect postoperative recovery of patients. Preoperative intra-peritoneal injection of sapylin is a valuable method in clinic.
