To reform the city's health service system, to start community health service, to establish a"six in one"regional network of health services including prevention, medical, health,rehabilitation, health education and family planning technical guidance which is functional reasonably, convenient, integrated is the inevitable trend of development of our country's health. To construction a community health service network must have a high quality information system, what's more, information systems and business process reengineering are complementary to each other. Information systems can not be truly realized if leaving the optimization of business processes and leaving the support of information systems, business process optimization is also very difficult to succeed.Objects: We increased, removed and altered some business functional parts of the com- munity health service information system in accordance with the specific circumstances ofWuhan City, and we had further reorganization of business processes to improve the funct- ion of community health service information system and the efficiency of community med- ical staff.Contents: 1.This paper discussed the theoretical foundation related to topics, including theories of the Community Health Service Management and Business Process Re-engineering. 2.We Used cluster stratified sampling , interviews and other methods had detailed to survey on status of Wuhan City Community Health Service Information System to find out problems and bottlenecks of system process. 3.We had system analysis and process reengineering in Wuhan City Community Health Service information, and used IDEF0 function modeling method to analysis the function of the system and business by the description of a series of systematic analysis, such as business processes charts and data flow diagram.Methods: We used cluster sampling method to survey 94 Community Centers and 362 community health service stations in Wuhan City. We investigated staff of community service centers and stations in order to obtain satisfaction of the current community health system. We will have functionality and optimization of business through surveys which identified function pares on low level of satisfaction.Conclusions: 1. Problems analysis, through investigation we believe that the function of the original Community Health Service Information System is not perfect, and it is difficult to carry on the function of referral management, moreover, it is necessary to strengthen the follow up control of chronic disease.2.Reengineering program, we reserved some parts which had high evaluation on the base of the original function of systems. According to the need of work, we optimized function parts of information service, referral management and follow up control of chronic disease management in order to meet the requirements about six-of-one service mode in community health.3.Effect evaluation, the community health service information system has reasonable settings of function in wuhan and the business processes are standard and concise. Furthermore, the efficiency of work in community has been improved and we received a good evaluation of users.
