Blackgrounds:Combined periodontal-endodontic lesions is common in the elderly population, there may be recurrent abscess and bone destruction, alveolar bone defect, eventually leading to tooth loss, seriously affecting the quality of patients'life. Clinical situation of these diseases is more complex, it has long course of treatment, and unsatisfactory prognosis.With the improvement of living standards and the increasing of life expectancy people also want to keep the teeth of some complex lesions. But those teeth treated only with conventional treatment, can not obtain satisfactory results, which is the thorny problem for dentists. We explore these cases during conventional treatment supplemented by surgical methods in order to save more and better teeth.Objective:We use conventional treatment supplemented by surgical methods to preserve teeth. and explore the effect of some complex cases, such as combined periodontal-endodontic lesions.Methods:We report 5 cases of surgical treatment used in the treatment of teeth preservation. Two cases of combined periodontal-endodontic lesions undergone tooth hemisection after root canal treatment; one case of combined periodontal-endodontic lesions undergone root separation after root canal treatment; one case of acute periapical abscess undergone tooth hemisection after conventional root canal treatment; one case with porcelain crown of chronic apical periodontitis undergone apicoectomy.Results:By observation of 1-3 months, we preserve teeth using conventional treatment supplemented by surgical methods, obtained satisfactory results.Conclusions:After comprehensive careful examination,well-developed treatment plan, and conventional endodontic treatment, the paper process the teeth with surgical methods such as tooth hemisection,root separation, apicoectomy, and then combine with restoration treatment. We treat 5 cases with integrated complex treatment, and preserve some seriously damaged teeth. Altough we obtained satisfactory results, long-term efficacy remains to be seen.
