Objective: To investigate the clinical effect of collagen membrane guided tissue regeneration combined with bone grafting in the treatment of periodontal endodontic lesions.Methods: To select the patients from the First Affiliated Hospital of Xinjiang Medical University Department of periodontal mucosa who had combined periodontal endodontic lesions from December 2014 to June 2016.There are altogether 13 people in this study,including 8 males(17 teeth),5 females(8 teeth),age from 39 to 61 years old,average age(50.92 + 7.71).The teeth after perfect root canal therapy and periodontal therapy still have deep periodontal pockets(PD = 5mm),bleeding on probing,pyorrhea or furcation involvement etc accord with periodontal surgery indications.25 teeth according to the wishes of patients divide into two groups,experimental group including 13 teeth,after root canal therapy and periodontal basic treatment,flap and graft Tianbo bone meal,then covered Hai'ao biofilm.control group including 12 teeth,after the root canal therapy and periodontal basic treatment,perform simple flap surgery.The patients were followed up for 6 months after surgey,recheck the operation area,and record the depth of exploration,attachment loss,gingival recession,loosening,and take the periapical film(the height of alveolar bone).Review to ensure the operation area bleeding index <2,according to the patient's oral hygiene,remove newly formed calculus and soft dirt.The clinical indexes of all teeth were recorded and statistically analyzed.All data were analyzed by SPSS17.0 software,P<0.05 means the difference has statistically significance.Results: the patients before surgey combined with after 6 months of surgey,the indexes of 2 groups were significant change(P< 0.05);compared with the flap group,guided tissue regeneration combined bone grafting group of periodontal pocket depth and attachment loss reduction,the alveolar height increased significantly,with statistical difference(P<0.05),while the gingival recession and tooth mobility are no significant difference between two groups.6 months later,X –ray showed that the density of the original bone defect area was increased,and there was no significant difference between the bone graft area and the surrounding tissue.Conclusion: the application of guided tissue regeneration combined with bone grafting in the treatment of combined periodontal endodontic lesions can achieve better clinical results than the simple flap operation.
