Objective:To evaluate the effect of distraction osteogenesis(DO) in the treatment of micrognathia associated with obstructive sleep apnea hypopnea syndrome (OSAHS).Methods:12 patients with micrognathia associated with OSAHS underwent DO, cephal-ometric measurements were made cephalometric films, and three-dimensional reco-nstruction were made based on the spiral CT before and after surgery, to evaluate the postoperative changes in upper airway and mandible.Results:All 12 cases had completed distraction. The length of distraction ranged from 12 to 30mm. The appearance were significantly improved.11 patients with OSAHS were cured and 1 was improved. All patients had varied degrees of occlusion disorders, but were basically normal 6-24 months after orthodontic treatment. In all cases, SNB angle, face angle, ANS-Me/N-Me turned out to be larger and Y-axis angle were smaller after surgery. The results were statistically significant. In all cases the median sagittal area of the upper airway, the horizontal area, upper airway volume, the volume of mandible got greater than the preoperative patients, and the results were statistically significant. Follow-up for 6-20 months, the position of mandible maintained stable, and the symptoms of OSAHS had no recurrence. Conclusions:DO is effective and safe for patients with micrognathia associated with OSAHS, and can be evaluated by cephalometric measurements, CT three-dimensional reconstruction and polysomnography (PSG).
