The Change In The Serum Levels Of Myeloperoxidase And Lipoxin A4 In Coronary Heart Disease Patients With Depression And/or Anxiety

PurposeTo investigate the changes and significance of the serum myeloperoxidase(MPO) and lipoxin(LXA4) in the coronary heart disease(CHD) patients with depression and/or anxiety.MethodsFrom December 2010 to February 2011,143 patients with CHD(CHD group) and 44 patients without CHD and depression/anxiety(control group) hospitalized in the Department of Cardiology of Second Xiangya Hospital were selected. The hospital anxiety and depression scale(HADS) was used to evaluate the psychological state of all subjects and the CHD patients were divided into depression/anxiety group and non-depression/anxiety group. The serum levels of high sensitive C-reactive protein(Hs-CRP), MPO and LXA4 were assessed, and the ratio of MPO and LXA4(M/L) was calculated. Above variables in three groups were compared and their relationship was analyzed.Result(1)In patients hospitalized in the Department of Cardiology, the incidence of depression/anxiety was 45.99%, in which anxiety was 36.36%, depression was 33.69%, comorbidity with depression and anxiety was 24.06%.(2) Compared with the control group, the serum levels of HsCRP, LXA4, MPO, and M/L ratios in both of CHD groups were significantly higher(all p<0.01). In the CHD patients, the levels of HsCRP, LXA4, MPO, and M/L ratios in the depression/anxiety group(4.50±3.26mg/L, 275.51±131.89pg/ml,482.66±186.75U/L and 1.85±0.51U/ng, respectively) were higher than the non-depression/anxiety group(3.85±3.03mg/L,252.11±127.49pg/mL,399.15±183.97U/L and 1.67±0.52U/ng, respectively), in which the difference of MPO and M/L ratio reached statistical significance (all p<0.05). (3)Spearman rank correlation analysis showed that MPO, LXA4 and M/L ratios had positive correlations with HADS-t, HADS-a and HADS-d score(all p<0.05). After controlling for confounding factors, partial correlation analysis showed that MPO was positively correlated with the HADS-t, HADS-a and HADS-d score; but the positive correlation between LXA4 and HADS-t, HADS-d score disappeared, changing to negative correlation; M/L ratio still had positive correlation with the HADS-t and HADS-d score, the correlation between LXA4, M/L ratio and HADS-a score disappeared.(4) The influence factors of the MPO, LXA4 levels and M/L ratio were analyzed by Multiple Ordinal Logistic Regression Model and the results were as follow:①LXA4, HADS-t score, unstable angina, acute myocardial infarction were the impact factors for the elevation of MPO (regression coefficients were 10.373, p<0.001; 2.026, p=0.031; 8.304, p =0.010; 8.676, p=0.005; respectively); There was no correlation between stable angina pectoris and MPO(regression coefficient 1.106, p= 0.680);②MPO was the impact factor for the elevation of LXA4(regression coefficient 3.247, p<0.001); stable angina was the impact factor for the decline of LXA4 (regression coefficient-4.351, p= 0.005); the impact of unstable angina, acute myocardial infarction and HADS-t score on LXA4 did not reach statistical significance (p= 0.145,0.452,0.190, respectively);③male, HADS-t score, stable angina, unstable angina and acute myocardial infarction were the impact factors for the elevation of M/L ratio (regression coefficients were 0.887, p= 0.045; 1.019, p=0.017; 3.551, p=0.002; 4.551, p<0.001; 4.606, p<0.001; respectively).Conclusion(1) The CHD patients hospitalized in the Department of Cardiology have a high incidence of depression/anxiety.(2) The pro-resolution factors increase with the elevation of the inflammatory response in vivo.(3) MPO may be a sensitive indicator for reflecting the state of acute inflammation, which is independent of HsCRP. (4) The inflammatory response in patients with coronary heart disease is accompanied by the presence of inflammation/resolution imbalance, especially in patients combined with depression/anxiety.(5) HADS-t score is an important factor for the inflammation/resolution imbalance in the patients with coronary heart disease.
