Objective To evaluate the value of imaging-guided percutaneous biopsy in the diagnosis of spinal lesions. Sum up the experience and discuss some clinical characteristics of percutaneous biopsy technology in spinal lesions.Method 422 cases of C-arm or CT-guided percutaneous spine biopsy performed between 2001-2009 were reviewed.Result The biopsy specimens were obtained satisfactorily in 419 of 422, drawing a success rate of 99.3%.32 cases were performed two times because of negative and unsatisfied outcomes.165 patients received surgical operation, which proved that 14 cases were misdiagnosis. Diagnose accordance rate was 91.5% between surgical and percutaneous biopsy results. The overall diagnostic accuracy was 93.6%.Conclusion Imaging-guided percutaneous spine biopsy technique charactered by safety, high efficiency, minimally invasive, high accuracy and low cost is an effective way in the diagnosis of spinal lesions. The source of metastatic bone tumor is determined by the combination of clinical manifestations, imaging and pathology. Sclerosing lesions should be alert for false-negative results, Performing the percutaneous biopsy again or open biopsy should be considered to confirm the diagnosis as far as possible.
