The Comparison Of Curative Effect: Treating LDH By Quadrant Expanding Piping System And Traditional Operation

Objective: The Intervertebral disc support the weight of the human body and the upper limband, in the daily life and work,it is The most vulnerable to strain.The reason for this is that blood supply is limited and less nutrition.Conventry report that The 20-year-old young man has disk degeneration,some people of the 20~30-year-old man appear apparent degeneration,the Fiber ring appear fissure.With the growth of the age,the vertebralpulp is radually narrowing and the Fiber ring gradually becomes thicker.All these lead to lumbar disc herniation .If the conservative treatment is invalid,the lumbar intervertebral disc patients require surgical treatment.The traditional operation don't requuire to destroy the zygopophysis,so it is not influential for the stability of the vertebral column. With the growth of awareness of minimally invasive, the question that how to reduce the patient's wounds and the postoperative complications is becoming more severe for the doctor.To discuss the difference of the effect between the Quadrant group and the traditional surgery group in treating lumbar disc herniation , summarize the clinical experience .Method: Select sixty patients of lumbar disc herniation ,with single nerve root involving symptoms, imaging showed single level herniation and no unstability and lumbar spondylolisthesis.All 60 cases were randomly divided into two groups, 30 cases received operations with Quadrant system in groupA, and the others with traditonal open window operation in group B. According to the visual analogy method (VAS), ODI standard and the improved Macnab standard, we evaluated the clinical outcome,and compared the index , including the length of incision, the bleeding volume ,the changes of the creatine kinase in preoperative and postoperative,and the time of getting up and hospitalization. Result: All cases were followed 8-23months.There were no significant differences between the preoperative ODI index, the scoring of one month later,and the final follow-up time in the two groups (p>0.05).The outcome effect of the 90% of the patients in Quadrant group is optimal,and 86.7%in the control group(B) was. There are no significance difference between the two groups's short-term effect (p>0.05). The VAS ratings showed significant differences between the two groups(P <0.05). The two groups have significant differences(P<0.05) in the incision length, bleeding volume , changes of the creatine kinase preoperative and postoperative and the time in bed and hospitalization.Conclusions:1The short-term curative effect of the Quadrant systemscores is similar with the traditional operation.They all acquired satisfied results, relieve pain, restore daily life.2The surgery trauma is smaller by using Quadrant system, and pain is lighter, recovering faster,the minimally invasive surgery applicating Quadrant system is a better way for lumbar disc herniation patients. In a word, the surgery applicating Quadrant system is microinvasive, it has better broad application prospect.
