Objectives:By gradually prolonging interval time of repositioning for patients, this study is to test effects of the clinically relevant duration of pressure loading(2h,2.5h,3h,3.5h,4h) on sacral and scapular skin temperature and pressure in subjects,which to determine whether gradually prolonging the reposition interval time method does not increase the incidence of pressure ulcers.Methods:Convenience sample of 64 patients from two nursing homes in Fuzhou from March ,2010 to January, 2011 observed by self-control study was chosen,who were long-term bedridden and non-independent replancement. Then the skin pressure and temperature of the scapulars and sacrococcygeal was measured, with subjects lying on the ordinary mattresses after different pressure loading (2h,2.5h,3h,3.5h and 4h).Every stage last 3 days.Meanwhile,the skin conditions and any pressure ulcers development were monitored.Data were collected concerning the general characterisitcs by the self-made questionnaire,patiences'pressure ulcers risk factors by Braden Scale for Pressure Sore Risk.The implementation of reposition nursing was recorded.Other nursing operating was as normal. The collceted data was analyzed with the statistical package SPSS11.5 by descriptive statistics, repetitive measure anova, chi-square, Pearson correlation inspection statistical methods. Results:1. There were five patients (7.81%) who were found hereditaria using gradually prolonging reposition invterval time method.2. By gradually prolonging interval time of repositioning ,comparing the sacral and scapular skin pressure,we found,there were no significant difference in the change of subjects'skin pressure under different interval time of repositionging (P > 0.05),wheras,there were significant difference about their skin pressure under the same interval time of repositioning(P<0.05).3. By gradually prolonging interval time of repositioning ,comparing the sacral and scapular skin temperature,we found,there were both significant difference in the change of subjects'skin temperature under different time interval of repositionging (P <0.05), and under the same interval time of repositioning(P<0.05).4.Pearson correlation analysis conducted over these data showed that the skin temperature of scapulars and the sacrococcygeal were significantly associated with the pressure loading time(correlation coefficient0.486, 0.506, 0.602,respectively).5. the reposition method of gradually prolonging the interval time can reduce the patients'pain and improve their comfort.6.Using gradually prolonging reposition interval time method (every stage lasts 30 minutes, to 4 hours/times), do not increase the incidence of pressure ulcers. Compare the results of assessment for the pressure ulcer risk factors in every stage,there were no significant difference, P > 0.05.Conclusion:1. Under these experimental conditions, the reposition method of gradually prolonging the interval time does not increase the occurrence of pressure ulcers, which manifests specially in the visual observation result and physiological measurement indexes.2. the skin temperature and pressure are taken as the importance reference of evaluating the degree of pressure tissues injury.3. the reposition method of gradually prolonging the time interval can improve the patients'comfort and sleep quality,and reduce the workload of nursing staff,which has clinical application value.
