Objective:Under the premise of without increasing the incidence of pressure ulcers, By thenursing methods of prolonging rolling over interva step by step, monitoring changesof the skin temperature and surface pressure value different time in main pressureparts of patients with the air cushion bed, exploring the differences of differentweight patients in different rolling over time interval.Methods:With the self-control study,selecting77patients who can’t change positionindependently and willing to join in this study from neurology randomly. Dividedinto low weight, normal weight, overweight according to body mass index. Usingself-designed questionnaire in patients with general information collection; Using the“Braden pressure sores forecasting” estimating pressure ulcer risk factors; Markingthe implementation of rolling over with a homemade "Turn over records". Byextending the rolling over time interval (extend30minutes every stage, extended to4hours/day)step by step to caring patients in air cushion bed, measured and recordedsurface pressure and skin temperature for2hours,2.5hours,3hours and3.5hours,4hours in patients about sacral rear and sides shoulder blade.Results:1.The patients of the skin appears abnormal in the research object is6,7.79℅.2.In the low weight group, compared with body surface pressure of both sides ofthe shoulder and sacral rear in patients using air cushion bed, rolling over in differenttime interval, with the extension of turnaround time interval, the pressure changes ofthe body surface has no significant difference (P>0.05) and the average values ofbody pressure was statistically significant (P <0.05) in sacral rear and both sides of the shoulder blades in the same turn time interval.3.In the low weight group, compared with the surface temperature on both sidesof the shoulder blade and the sacral in patients using air cushion bed rolling over atdifferent time interval, finding the result of surface temperature changes were nosignificant difference (P>0.05) with the extension of turnaround time interval, Theaverage temperature value is statistically significant (P <0.05) n the same turn timeinterval on sacral rear and both sides of the shoulder blades.4.In the normal weight group,the change of the surface pressure was nosignificant difference (P>0.05)on both sides of the shoulder blade area and sacralwtih rolling over in different time interval. The average values of body pressure wasstatistically significant (P <0.05)in the same turn time interval on sacral rear and bothsides of the shoulder blades.5.In the normal weight group, the surface temperature changes have nosignificant difference (P>0.05) with the extension of turnaround time interval,rollingover in different time interval on both sides of the shoulder blade and the sacral.6.No obvious difference of the pressure (P>0.05) on the body surface is found asthe turnover interval prolongs, through the comparison of the pressure on the scapulaand the sacrum when overweight patients turn over in the air bed after differentintervals.7. There is notable difference in shell temperature after lying on back for4hours(P>0.05) as the turnover interval prolongs, through comparison of the shelltemperature on the scapula and the sacrum when overweight patients turn over inthe air bed after different intervals.8. It is found in the research that the gradual prolongation of interval is aneffective way to reduce the pain and discomfort caused by turning over, so that thesleep quality can be improved (P <0.05).Conclusions:1. The use of air bed has the effect of reducing the pressure on the main pressedareas. The method of gradual prolongation of turnover interval has the same effect ofpreventing pressure sores with the traditional turning-body-over methods. 2. There is difference in the turnover intervals for patients of different weight whenthey use air beds. Patients who are underweight can turn their body over once as longas every3hours, while those who are overweight or of normal weight every4hours.3. The gradual prolongation of turnover interval has certain promotion valuebecause it can decrease the frequency of turning over, raise comfort level as well asreduce the workload of nursing staff.
