Exploration Of The Syndrome Distributing And Medication Characteristics In 412 Primary Gout Patients

ObjectiveOur study is to explor the characteristic of general condition, symptoms, Syndrome and medication characteristics, who hospitalized as gout in the 1st affiliated hospital of guangzhou university of traditional Chinese medicine. In order to find further regularity of gout at the contemporary era, and to give better clinical theoretical basis and guiding significance of syndrome differentiation and treatment.MethodsWe summarized medical records of patients of First Affiliated Hospital of Guangzhou University of TCM from 2001 to 2009 that are diagnosed as gout (412 cases):summarized raw information of sexuality, age, birthplace, pathogenesis, years, season, intercurrent disease, joint symptoms, systemic symptoms, tongue and pulse, level of serum uric acid, TCM syndrome, Chinese traditional medicine etc, and draw the conclusion of distribution law.ResultsThe incidence rates of Gout in male are more than those of female. Age distribution of Gouts are most frequent in 50～80, when the age of 17 is the youngest. And before 70, the incidence rates of Gouts is increasessing with the growth age changes.Guangdong Province is the highest incidence area of Gout, following is Hunan province, fujian province and hainan province, shandong province, henan province, hubei province, etc. the patients of gout in our hospital increased remarkably after 2003. and the highest number of gout remarked was in 2009, which was 95 cases (23.1%). Gout can break out in four seasons, when spring is the most frequently season. The most pathogenesis are within 10 years and 1 year-10 years, sum to 195 cases (of 47.3%), within 10 years is less in the course. the master inducement contain no obvious cause, high purine diet, drinking. Gout intercurrent disease ratio:merger of hypertension is 36.2%, merger of diabetes is 17.7%, merger of dyslipidemia is 11.7%, merger of coronary heart disease is 7.0%. Analysis the laboratory test results in hospital:Serum uric acid higher than normal amount (>=430umol/L) total of 277(67.2%), the scope of the normal (<430umol/L) total of 127 (30.8%).The most pain part are metatarsophalangeal joint, total of 238(57.8%), followed by ankle joint, foot back joint, knee joint, metacarpophalangeal joint, wrist joint and elbow joint,etc.The most remarkable Systemic symptom is Sleep disorders. Expression of Patient tongue is that the most of red tongue and red tongue tongue fur, followed by pale tongue and slight yellow tongue fur, except that has a lot of blood stasis, such as Ageless red tongue Light ageless tougueand dark etc. In all TCM syndrome type, the first of type is damp and hot accumulate knot card, which have 195 cases(47.3%).The second is damp and hot accumulate knot card or Stasis hot block, which basis on deficiency of the liver and kidney is total of 74 cases. Various types of traditional Chinese medicine mixed together, damp-clearing drugs is the most of frequency of use, followding are clearing medicinal drugs, Remove blood stasis drugs, Fill empty drugs, rheumatism frugs, turn wet drugs, etc.Conclusion1. Gout patients in our hospital are more of middle-aged male. The tendency of making gout incidence younger. Increased significantly of gout incidence in recent years, to be in hospital most in spring.2. The most gout incedence of area is Guangdong Province, which it consider are geography, climate, diet, humanity and so on. Gout patients intercurrent diseasemerger is higher than Normal Person, such as hypertension, diabetes, dyslipidemia, coronary heart disease.3. Joint symptoms display that it's basically about the joint swelling heat pain. The tongue demonstration means humid heat, later hurt genuine YIN of liver-kidney, last, qi-stagnancy and blood stasis pathogenesis occur.4. TCM syndrome most are damp and hot accumulate knot card, and damp and hot accumulate knot card or Stasis hot block, which basis on deficiency of the liver and kidney.5. Follow the basis for differentiation of syndromes and disease, the traditional Chinese medicine most of permeability wet benefit water, clearing heat, clearing damp.
