Objective To investigate the molecular mechanisms of diabetes mellitus to the cultivation of regenerated bone tissue forms, fibrous tissue and bone regeneration on implanted rat models, and as well as the effects of Yi Qi Bu Shen Huo Xue prescription-Qi Lu Dan on the diabetic bone integration of implants. In addition, to observe the clinical resuls on the diabetes taking Qi Lu Dan after implants.After restablishing the diabetic rat with implant in bone model, dosed them in every group (control group using saline, Western medicine group with suspension metformin hydrochloride, Chinese medicine group with Yiqibushenhuoxue Prescription, Chinese and Western medicine combining group using suspension metformin hydrochloride and Yiqibushenhuoxue Prescription). After 12 weeks, made HE stained tissue sections, and measured by light microscopy and bone parameters, and then compared Yiqibushen Recipe-Qi Lu Dan with aminoguanidine in diabetic rats implanted by titanium implants at improving effects on bone intergretion.9 cases of diabetic patients having dental implants were also clinically observed to see the situation around the implants after taking the Qi Lu Dan.Results The fasting plasma glucose of rats injected streptozotocin (STZ)was significantly higher after 14ds,92.3% modeling success rate. During the process, there was no significant difference among the Western medicine group(WG), Chinese and Western medicine combining group (CWG)and the normal group(NG) in FBG (P>0.05), but traditional Chinese medicine group(CG) was different to NG in FBG (P<0.05). Light microscope (12 weeks after implant placement):bone plates in CMG had better continuity, thicker, smooth surface, and a large number of osteoblasts can be seen around as well as larger bone cell density. The bone plate in CM also had better continuity, thicker, smooth surface, a certain amount of osteoblasts, and good bone cell density. The bone plates in WG was thiner, better continuity, a large number of osteoblasts and larger bone cell density. Comparison of bone measuring parameters (after 12 weeks implantation):Between WG and CG:the width of bony plates and number of bone cells in planting and non-planting areas in WG were lower than those in CG (P<0.05); The number of osteoblasts in WG was also lower than CG(P <0.05). Between CG and CWG:the width of bony plates and the number of bone cells in planting and non-planting areas were not significantly different to control group (P> 0.05); and the osteoblasts of treatment group in planting and non-planting areas were less than that of control group (P<0.05). Between WG and CWG:the width of bony plates, the number of osteoblasts in planting areas and bone cells in planting areas and non-planting were all significantly lower than treatment group (P<0.05). During clinical observation, Qi Lu Dan can improve the tissue situation around the implant, without significant adverse reactions.Conclusion Yi Qi Bu Shen Huo Xue prescription-Qi Lu Dan can improve the bone combinition of implants, but fail to lower plasma glucose completely to normal; hypoglycemic agents with Qi Lu Dan can effectively promote and enhance bone combinition through regulating plasma glucose and bone metabolism, better than using Western or Chinese medicine treatment only.
