The Curative Effect Of The Lishangxiaozhong Formula Treating The Post-surgery Swelling Of Fractures Around The Knee

Purpose:To study the curative effect of the Lishangxiaozhong formula treating the local post-surgery swelling of fractures around the knee, so as to evaluate the detumescent and acesodyne effect and the after-operation functional rehabilitation effect of the formula. To discuss the TCM mechanism of the Lishangxiaozhong formula, so as to provide theory evidence for the detumescent clinical effect of empirical formula and the application of TCM formula of activating blood circulation to dissipate blood stasis for the treatment of fractures.Methods:Adopting the activating blood circulation to dissipate blood stasis research methods, the author selects from those inpatients who have been performed traumatic orthopaedics operation in the First Subsidiary Hospital of Guangzhou University of TCM from June,2010 to April,2011 and divides 32 patients who according with introduction standard and elimination standard into two groups in random:16 cases in therapy group(using the Lishangxiaozhong formula) and 16 cases in control group(using the Taohongsiwu formula). Both two groups received regular treatments like antibiotics for 3-5 days, prophylactic, supportive treatment, symptomatic treatment and so on within one week after the surgery on the fractures around the knee. In addition, The therapy group also received the treatment of Lishangxiaozhong formula(1 dose, bid×1 week) while the control group took the treatment of the Taohongsiwu formula(1 dose, bid×1 week). To make a research of the change of exercise pain and swelling on the knee joint within one week after the operation of the two groups and to make a record of the WBC count, CRP value and HSS evaluation on the knee joints on the seventh day before and after the treatment, the author synthesizes all the observed indicators, applies the statistical analysis and gains the statistics conclusions.Results:Using the Repeated analysis of variance, the author finds that the two groups shows no significant differences (P>0.05) on the exercise VAS marking and the swelling condition of knee joints on the first and third day of treatment, while shows significant differences (P<0.05) on the rest two days. After the T-test, the two groups shows significant differences (P< 0.05) on exercise VAS marking on the seventh day while no significant differences (P＞0.05) on the third and fifth day both before and after the treatment. The two groups have no notable differences (P>0.05) on the basal circumference numerical value of the unaffected side around the knee joint, no notable differences (P＞0.05) on the swelling conditions of the knee joint on the first and third day both before and after the treatment while significant differences (P<0.05) on the fifth and seventh day after the treatment. For the inflammation reaction, the two groups show no notable differences (P＞0.05) on the WBC count and CRP value on the first and seventh day after the operation while shows significant differences (P<0.05)on the first and seventh day after the treatment. The two groups share no significant differences (P＞0.05) on the HSS evaluation on the first day after the operation while share significant differences (P<0.05) on the seventh day and share significant differences (P<0.05) on the first and seventh day after the treatment.Conclusion:1. The Lishangxiaozhong formula can effectively and stably relieves the pain after the surgery of fractures around the knee and during the functional rehabilitation exercise.2. The Lishangxiaozhong formula can effectively reduce the local swelling after the operation of the fractures around the knee.3. The Lishangxiaozhong formula can effectively accelerate the functional rehabilitation after the operation of the fractures around the knee, by ameliorating the pain and swelling on the knee joint and boosting the enthusiasm of the patients concerning functional exercise so as to improve the functional rehabilitation effect.4. The Lishangxiaozhong formula produces little side effects and shows high safety on the clinical application.5. The Lishangxiaozhong formula shows no definite function for reducing the inflammation reaction and requires further study.
