BackgroundConstitutional types of traditional Chinese medicine(TCM) are important representations of human life, which reflect the particularity of psychological response in physiology and the tendency of the onset of the illness in pathology. The promulgation of the first Classification and Judgement of the Traditional Chinese Medicine Constitutitons provides a basis for the study of standardization of TCM constitutitons.It has the important real significance to study the constitutional characteristics of people with primary hypertension and improve the pathology constitution actively. High homocysteine (Hey) is an independent risk factor for stroke and the coexistence of hypertension and high Hey has significant synergetic enhancement on cardiovascular and cerebrovascular events.Presently, the studies on relationship between constitutional characteristics of people with hypertension and factors for stoke have already been reported frequently in China, but the study of the relationship between constitutional characteristics of people with hypertension and high homocysteine has not been reported.ObjectiveTo investigate the relationship between the TCM constitutions of people with primary hypertension and Hey, and explore the predictive value of TCM constitutions in Hey level of patients with essential hypertension. so that, it can provide the basis for the improvement of the pathology constitutions and earring out the TCM interventions in "H-type hypertension".Methods The study was based on China Stroke Primary Prevention Trial (abbreviated as CSPPT). The cases of mild to moderate hypertension were from the CSPPT third follow-up survey in Donghai County, Lianyungang City, Jiangsu Province from 2010-1-15 to 2010-2-1. The survey of TCM constitutitons was performed by standardized TCM Constitution Questionnaire.Investigators asked patients item by item and filled out the questionnaire, then judged the patients's constitutions. Used the univariate analysis, multiple linear regression and the principal components and regression analysis to analyze the influence of TCM constitutitons on the Hey level. Epidata 3.1 was used for double data entry and SPSS 13.0 for data analysis.ResultsAmong the survey of 1232 cases of mild to moderate hypertension, the frequency of pathology constitutions was 66.8%. Yang deficiency constitution accounted for the highest proportion of pathology constitutions, followed by Yin deficiency constitution, Qi stagnation constitution, phlegm-dampness constitution, Qi deficiency constitution, Blood stasis constitution, Damp-heat constitution and Special constitution.862 cases had blood homocysteine test results, among which,45.9% patients were hyperhomocysteinemia. The univariate analysis showed Qi deficiency constitution score, Qi deficiency constitution score and Qi deficiency constitution score was correlated with the homocysteine level, but correlation coefficients were very small. Extracted principal component, and the regression analysis was carried out, taking factor scores as as the independent variables, and Hey level as dependant variable. The effect of factors associated with the constitution on Hey level had no statistical meaning.ConclusionThis study has not found the the relationship between the TCM constitutions of people with primary hypertension and Hey level. But this study is a follow-up survey, and all the cases of mild to moderate hypertension are from the Donghai County, Lianyungang City,Jiangsu Province, the study sample has obvious limitation. Consider the sample correlation coefficient does not directly reflect the the degree of correlation between two variables, it is not possible to obviate the relationship between the TCM constitutions and the Hey level in mild to moderate hypertension, this problem will need to be further gone into.
