Study DesignA prospective clinical case study.Object iveTo investigate the clinical effect of the patients with lumbar disc herniation(LDH) following the treatment of tonifying Kidney and removing blood stasis.Summary of Background DataLumbar disc herniation (LDH) is a common and frequently occurring disease of Orthopedics, the most common cause of low back pain, which has brought great physical and mental suffering for patients, and has brought huge medical expenses for society and person. Chinese medicine treatment is an important conservative treatment method for lumbar disc herniation (LDH), which has less cost, less side effects, effective, easy to accept by patients, and rare complications. Currently Chinese medicine treatment in lumbar disc herniation (LDH) has shown good signs, and has broad market prospects.MethodsWe prospectively studied 62 patients with lumbar disc herniation (LDH) from April 2010 to April 2011, who had treatment in spinal specialist out-patient of Guangzhou University of Traditional Chinese Medicine First Affiliated Hospital.62 patients were randomly divided into Chinese medicine group and Western medicine group. Among them,30 cases of Chinese medicine, took traditional Chinese drugs with tonifying Kidney and removing blood stasis, 1 day service; while 32 cases of Western group, took Celecoxib capsules (Celebrex), A 0.2g, day one. Parameters include patients'age, sex, lesion segment, course of disease, the severity of lumbar disc herniation, evaluation of the efficacy of treatment, and the score changes of visual analogue scale (VAS), Oswestry functional disability index score, lower back pain JOA score before and after treatment.ResultsAll 62 patients completed 3-day,7-day and 15-day follow-up. After treatment, two groups of patients'symptoms were significantly improved effective rate after 3 days'treatment was 83.9%, effective rate after 7 days'was 95.2%, effective rate after 15 days'was 96.8%. Before treatment, the average visual analogue scale (VAS) of all patients was 8.2 points, after 3 days'treatment, it was 6. lpoints,3.8 points after 7 days',2.3 points after 15 days'. Before treatment, the average Oswestry Disability Index score of all patients was 33.8 points,17.5 points after 7days'treatment,11.1 points after 15days'. Before treatment, the average JOA score of all patients was 11.5 points,19.3 points after 7days'treatment,22.4 points after 15days'. The VAS scores, Oswestry Disability Index score and lower back pain JOA score of the two groups after the treatment, compared with those before treatment, the difference was statistically significant. Comparing with the two groups'effective rate after 3 days',7 days',15 days'treatment, there was no significant difference between the groups. After 3 days' treatment, Chinese medicine group's VAS score was 6.53±1.93 points, Western medicine group was 5.63±2.12, differences between the two groups were statistically significant (P=0.04). Before the treatment, after 7 days' treatment and after 15 days' treatment, the VAS scores, Oswestry scores, and lower back pain JOA score between the two groups have no significant difference.ConclusionAfter the treatment, Chinese medicine and western medicine group patients'VAS score, Oswestry Disability Index score and lower back pain JOA score compared with those scores before treatment were significantly improved. Compared with the Chinese medicine, the VAS score of Celecoxib group after 3 days'treatment decreased more significantly, there was significant difference between the groups, which illustrates that Celecoxib capsules have more rapid relief in acute pain. Compared with the Western medicine group, the efficacy, Chinese medicine groups' VAS scores, Oswestry score, lower back pain JOA score of 7 days and 15 days, were not statistically significant. The results show that patients with lumbar disc herniation using the treatment of tonifying Kidney and removing blood stasis is obvious effect, Pain, numbness and other symptoms are relieved, exercise capacity is restored, and the quality of daily life is improved.
