Theoretical studyDiscussing and analysing TCM disease name, etiology and pathogenesis, syndrome differentiation and the definition of modern medicine, epidemiology and pathological mechanism through systematic review of theoretical literature on heart failure, to lay the theoretical basis for clinical study. Clinical experimental studyObjectiveTo study the relationship between T-381C polymorphism in BNP gene and incidence of chronic heart failure (CHF), and syndrome differentiation type of Traditional Chinese medicine (TCM) in patients with CHF, in order to provide a new theoretical evidence on individual prevention or treatment of CHF and impersonal gist for syndrome differentiation of TCM.Methodsâ‘ Case-control study, case group, a total of 94 in-patients admitted from January 2010 to January 2011 with definite diagnosis of CHF were collected, while the control group, a total of 50 individuals for health examination with age and sex frequence matched were also collected. By constructing the clinical epidemiology questionnaire, retrospectively collected the clinical data of two groups, EpiData 3.1 software for data entry and check, SPSS 18.0 for statistical analysis.â‘¡Polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP) was used to analysis the BNP gene T-381C polymorphism, and agarose gel electrophoresis was used to identify the genotype. Gene counting method was used to calculate genotype and allele frequencies, Hardy-Weinberg law was used to detect population representation and the different distribution of BNP genotype and allel frequencies was used to investigate the relationship between the BNP gene T-381C polymorphism and occurrence of heart failure. Non-conditional logistic regression analyse was used to screen risk factors of heart failure.â‘¢Analysis on TCM syndrome type distribution of CHF. Compare the clinical data of different BNP genotype. The possible association between the BNP and the TCM syndrome type in CHF was tested by x2 tests.Resultsâ‘ Comparison of general condition between CHF group and control group, the systolic blood pressure was significantly higher(P=0.0028) in CHF group, but the diastolic blood pressure was not statistically different(P>0.05).The body mass index, lymphocyte count, RBC count, hemoglobin, hematocrit and high density lipoprotein cholesterol in CHF group were significantly lower than those in control group(P<0.05),but the blood glucose, uric acid, urea nitrogen, creatinine, aspartate aminotransferase was significantly higher(P<0.05). Other statistical indicators of two groups showed no significant difference(P>0.05).â‘¡Neither the genotype frequencies of TT, TC+CC (x2=0.7497, P>0.05) nor the allele frequencies of T and C (x2=0.7711, P>0.05) differ significantly between the CHF group and control group. The risk factors of heart failure were studied by non-conditional Logistic regression analysis. The results showed that hypertension, coronary heart disease, CAD family history, high density lipoprotein cholesterol (HDL-C) were closely related to CHF. HDL-C was the protective factor (OR<1), the remaining three were risk factors (OR>1).â‘¢The distribution of TCM syndrome patterns were:qi-deficiency, blood-stasis and phlegm-fluid retention type was most common, which took the proportion of 19%. Then were qi-deficiency and blood-stasis type and deficiency of both qi and yin, phlegm and blood stasis type. The triglyceride level of TT genotype was significantly higher than that of TT+CC genotype(P<0.05). There was no significant difference in the distribution of TT genotype and TC+CC genotype and the allele frequencies of T and C in various TCM syndrome types.ConclusionsThe T-381C polymorphism in BNP gene is not associated with the occurrence of CHF. The BNP genotype might not be related to TCM syndrome type of CHF.
